Brief Answer:
Yes, you should be concerned.

Detailed Answer:
Hi,

Welcome to the health care magic. I am Dr. Ganesh.

In a known case of dementia if the patient complains of pain in the both arms - I would recommend you to get him to ER and get the vitals checked like BP, Pulse,ECG for cardiac pathologies.

Basically the disease dementia mostly due to small vessel ischemic / insufficiency in the brain due to underlying medical conditions and age related vascular occlusion.
So possibilities of vascular insufficiency can be suspected, please take him and get basic things checked. That will help him.

Some times the patient may be just confused, but one can not take any risk. Good luck.

Any thing to ask ? do not hesitate.
Thank you for approaching health care magic.
Wish you good health .
