Suggest Treatment For Cough And Throat Irritation

Dear Doctor,
My wife is 66 years old.She has throat congestion since 20 to 25 rs. We have seen many doctors and taken many medicines .But no use. She often gets flum in between the lungs and the throat and when she tries to cough it up it doesnt comeout. she goes on coghing at last for a few minutes and after half an hour or so she gets the flum and when spits out. she will be fine.The flum is also very very little in the size of letter " O " she is gets often cough, throat irritation,cold,nasal blockage and at times fever in all these years. She has undergone scan for sinusitis. but it was not there , we are told. Chest x ray pa view was taken (23/8/2016). It says Heart and Iorta are normal,Both lung fileds are clear,both hilia, costophrenic angles and domes of diaphgragm are normal.Bony cage is normal,
As a lost resort i am referering to you.Kindly advise

By your history and description, possibility of allergic rhinitis and allergic bronchitis.
Cough, throat irritation, cold, nasal blockage etc are symptoms of allergic rhinitis and allergic bronchitis.

Best treatment of these are
1. Intranasal spray containing steroid (fluticasone)
2. Inhaled bronchodilators (formoterol or salmeterol) and inhaled corticosteroid (ICS) (budesonide).
You have mentioned drugs she had tried. She didn't try any nasal spray or inhaler.
So better to start intranasal spray and inhaled treatment.

She will mostly improve with all these. Drink cleaner water and exercise regularly.

I forgot to mention that at times she used Nosivion nasal spray. she is diabetic since 2005 and had Thyroidsm since 3 years .Will there be broncho dialator inhalers in the market? Is it to be inhaled when ever the symptomms occur or she has to inhale every day? You have suggested steroids. Dont they harm the patient if used for long? Kindly advise

I can understand your concern.
Nasivion nasal spray is having oxymetazoline. It is not steroid. It is decongestant. It is for temporary relief. Fluticasone nasal spray is the treatment of choice for allergic rhinitis.
Intranasal steroids and inhaled steroids are locally acting steroids. They are not going in systemic circulation and hence devoid of systemic side effects. So inhaled steroids are very safe for long term use. And patient should take it daily, not as and when required.
And yes, they are easily available in market.
I don't know whether you will get it without prescription or not.
Commonly available budesonide inhalers are budecort and budate.
