Suggest Treatment For Constant Ache In Right Kidney Area

Question: I have been taking Nexium for the last 4 days to help a bleeding stomach as had a bad reaction to Advil. However, I have had right kidney pain since then for the last 2-3 days. It is very uncomfortable and painful. My urinating has improved but the pain in the kidney has not. Now I just developed a very dry mouth. I am worried both about Kidney infections as well as the possibility of a blood clot traveling to my lung. Is this possible, as the pain is mostly in my right lower back?

I appreciate your concern

NSAIDs like ibuprofen ( Advil ) commonly lead to Gastrointestinal ulceration and sometimes upper GI bleeding. PPIs like esomeprazole (Nexium) at optimum dose are helpful in healing of the ulceration. If the bleeding persists or recurs then an upper GI endoscopy should be advised to localize the bleeding focus and look for other causes such as a mass or ulcer that requires a biopsy. Hepatitis profile, blood sugar, urinalysis, blood counts and metabolic profile testing should be done as part of the workup. Your current symptoms warrant an EKG and an ultrasound abdomen pelvis to be done. If you don't have a DVT or coagulopathy then its unlikely to be a clot leading to pulmonary embolism. The pain you described either related to a small stone in the urinary tract or gall stone causing cholecystitis. However , a cardiac cause should always be ruled out primarily by doing an EKG. Antispasmodic medication like hyoscine butylbromide can be advised if its indeed renal colic.
