Suggest Treatment For Complete ACL Tear In Knee

Question: Hi i had fallen down in XXXXXXX 2014 from two wheeler , i had twisted my knee , felt dizzy and couldnt walk immediately. I was limping. Initially the first care i got was spraying a pain reliever. It was slightly swollen. Even after two or three days i was limping and couldnt walk with pain. I was wearing knee band. I consulted a ortho doc who examined my leg and prescribed rest and some medicine. He told me to come after one week if the pain persists. I am a badminton player , not a professional though and he told me not to play. So i didnt play for 4 months.

The i started playing without strainig my leg from May. From then , i have faced problems in my leg while playing, twice i have fallen down , with pain in the knee and it will be alright after few days. Sometime it took jus oen day or so to be fine.Sometimes i would not fall but jus twist or strain that leg which would pain. I consulted few doctors in between who told me to strengthen my quadruples , and also suggested to take MRI.

Recenty one week back i again injured the same leg, i was quite playing well for few months before this incident without any kind of pain. This time the pain didnt subside by one day . I also had a match in two days. So i jus put a knee band and played the match. I ddnt have much pain but still it wasnt fully gone. after the match i have stopped playingandresting my leg. But still the pain hasnt subsided and i feel a little uncomfortable while walking up or down the stairs. this time the pain is different it is paining below the knee. And now after few days i feel a sharp shoot of pain inside the knee, not sure if that is my imagination.

I took an MRI and it says Near compelete tear of ACL. But another doc i visited after said it is hamstring injury now. I told him all this history too. I am not sure how to proceed

I interpreted your entire query and understand your concern.

This is extremely unlikely to be a hamstring injury. Your knee clinical picture straightway points to ACL tear.

Let me explain each one in detail-

1.Hamstring injuries usually occur with sudden hip flexion as in Skiing injuries. and in athletes who do not warm up.

-A hamstring injury is agonizing, making it impossible to walk or even stand.

-Pain hamstring injury is always located in the back of the thigh and lower buttock when walking, straightening the leg, or bending over.

-All the above features do not match your clinical profile to say it as Hamstring injury.

2.Anterior cruciate ligament (ACL) injuries are most often a result of low-velocity, noncontact, deceleration injuries and contact injuries with a rotational, twisting, valgus stress, or hyperextension of the knee or leg. You have initiated injury through a fall from the bike -later this worsened by repeated sporting activity and falls.

Following features in your case can help to formulate the diagnosis of Complete ACL tear-

1. Pain and inability to continue an activity.

2. Swelling and instability of the knee, with frequent relapses of joint discomfort.

3. Development of a large knee swelling( due to blood collection) immediately after trauma.

4. MRI has a sensitivity of 90-98% for ACL tears. MRI also may identify bone bruising, which is present in approximately 90% of ACL injuries.An MRI allows the physician to confirm an ACL tear.

5. MRI study in your case is self explainatory.

Complete ACL tear can be managed in your case through Surgical intervention only.

Non operative management/NOM is considered only in elederly and those who do not indulge in sporting acitivities.Hence this NOM is not an option for you.

Intra-articular reconstruction of the ACL has become the criterion standard for treating ACL tears.

You need to consult an Orthopediacian who has surgical expertise in treating Complete ACL tears.

I strongly advise you to seek an early intervention for Complete ACL tear to prevent any further deformities, that may interrupt with your playing skills.
