Suggest Treatment For Cold, Fever And Persistent Cough In Children

Question: Our 1.5 year old daughter had a cycle of cold, fever and is left with a persistent cough for over a week. She is paranoid and cries a lot of putting a mask on for nebulization of Asthalin recommended by a doctor. Also we are hesistant to start Omnacortil steroid What are our options to administer Asthalin and avoid steroids ?

Asthalin contains salbutamol a bronchodilator. It can be given by either oral or inhalation or both. The choice for mode of administration depends on the severity of clinical symptoms. Nebulisation works faster with longer duration of action and with better peak concentration. The side effects if any are also less in nebulisation.

So If there is severe spasm then nebulisation is a better choice. Most of the children don't like nebulisation and may get highly irritable at that time.

The newer prescription suggests that this is an acute exacerbation of hyperreactive airway disease. What you can do that decrease the frequency of nebulisation and also give oral asthalin syrup after consulting your physician.

I know an educated person usually get afraid of using steroid. But steroid can be given for short duration(5 days) without any significant side effects. This is the treatment protocol as steroid decreases the spasm by decreasing inflammation and helps in early recovery. Give omnacortil with food to avoid some gastric side effect.

Montair helps in controlling the exacerbation for some longer duration and usually given for two to three weeks.

So I would suggest you to consult your physician and tell him/her about the irritability during nebulisation. Discuss for the oral asthalin with decreased frequency of nebulisation or oral asthalin only if spasm is not severe.

Dr XXXXXXX Thank you for the quick response. It answered my specific concerns. I am puzzled why my doctor did not even mention oral asthalin option. Due to wide variation in standards zero traceability and ,worrying track record of ethics of practicing medicine in our country we are forced to use services like these to make informed choices. Are the medicines mentioned earlier known to react with homeopathic medication?

Hello Sir,

I am sorry, but as in our professional studies we were not given any idea about homeopathic medicines I wont be able to answer it exactly.

You have not mentioned that for what these homeopathic medications are being taken. If you are giving it for the same condition then stop it immediately as both may have same mechanism of action and that can be taken care of by allopathic medication only. If you are using it for some different condition then also I would advice you to stop the homeopathic medicines till the time she is on current medication as the nature of interaction is difficult to assess.

I have noted one important history that the weight of the child is only 8 kg which is lower than the normal limit for this age. There are so many questions in my mind regarding this, was she born premature, any significant illness she was suffering from, is there any loss in weight, is she on proper diet etc.

Please let me inform about this.

I would advise you again to discuss about the nature of medication with your physician (regarding oral/nebulisation).
