Suggest Treatment For Burning Sensation In Skin And Penis, Fever And Body Pain

Question: Hi doctor. Actually I had unprotected sex on April 20th 2014. After two weeks I got fever and body ache. I went to doctor Suddenly I had tablet and injection. From tat tym it started burning sensation in my skin and pennis heavily. I am so afraid and tolerated for almost till September 20 2014 (120 days). Finally I went for HIV 1&2, p24 test on septeber 20th.but it shown non reactive. Still I am having severe burning sensation which I mentioned early. Please tell me wat to do. Whether I have to go HIV test again. Please help me out.

Hi,

From what I understand you have an ongoing burning sensation of the skin and penis after an episode of unprotected sexual intercourse since April 20th. Your concerns about HIV in this case is not rational as:
1. Burning paraesthesia is not specific symptom of HIV
2. You have had a negative HIV tests after 3 months of exposure.

So be assured you have no indication of being HIV infected. To make yourself more assured, I will suggest you to get another HIV Elisa test (not P24 combo) test done now (i.e. at 6 months). Besides HIV test, I will also order for complete blood count, blood sugar as well as vitamin b12 check up. You should also consider consulting a counsellor at ITCT centre nearby.

I feel shame and afraid for another HIV test. Actually after exposure on 20th April 2014. I got fever and body ache after 2 weeks, were I went to doctor and had a medicine. But nothing cured and burning sensation started. I waited for 2 weeks, again went to another were I have complete blood count, fever and urine test. Then doctor told I have typhoid and jaundice. He suggested me typhoid tablet for 1 week and after 1 week I went through same test again with my own, but everything is OK. Finally I went through HIV test after 120 days of exposure. It came non reactive. But still I have burning sensation in my legs, pennis, stomach and some part of body.

Please reply for this. Suggest me wat to do and suggest me some doctor in XXXXXXX or else can I get a appointment with u. I need my life. Please help me

Apologies for the late reply. I was away from internet during the weekend.

I went through all the reports and all of those were unremarkable. It didn't reveal any finding to explain your burning sensation. Further and most important, HIV test was truly negative. History until this point of time has revealed to us that chances of you becoming HIV positive after the last test are extremely poor. So put asides fears of HIV and work with your doctor to get paraesthesia treated.

I suggested a second test as a protocol. However I don't expect it to show up positive. If I were you, I will go to the laboratory without fear and confident that the next HIV test will not come, I repeat will not come positive. So don't panic and get the test done. You can alleviate all thoughts about HIV from your mind and leave in peace.

As far as the burning sensation is concerned, many a times we are unable to find a precise organic cause. Vitamin B12 assessment is one test that I routinely do on such patients. A low vitamin B12 is one common reason for it and correcting it will to an extent solve this burning sensation. You can get this test done along with HIV antibody (Elisa test).

The fever and the typhoid infection which you had closely after the sexual event was most likely incidental. It has nothing to do with that sexual exposure or has any relation to you becoming HIV in future. I am glad you have no more fever now.

In a nut shell, based on your history and the available reports, the possibility of HIV infection is almost nil. Get the second HIV test and vitamin B12 assessed. Consider counseling, it is extremely helpful to treat the anxiety about HIV which you have got. It might also help in treating the burning sensation (you should try it before considering it useless). If they don't, a small dose of antianxiety medication/SSRI can be used.

But most of the hospital going with Anti HIV 1&2(Ag/Ab)&P24 by cmia. Is this test is OK. Or some other test is there for HIV.whether I can go with the test again which I made earlier or different HIV test. Suggest me please.

Hi,

There are few varieties of test available to test HIV. The test you had performed earlier has two components - HIV antibody detection as well as P24 antigen estimation. P24 antigen estimation has little relevance after 4 weeks of exposure. Therefore I suggest we test only the antibody.

The test is called HIV 1 and 2 antibody (ELISA) test. I am sure all laboratories will know about it. You can live in peace after this test. I am sure you will be able to live peacefully once you get this result.
