Brief Answer:
CT scan and actual examination

Detailed Answer:
Hi Sir.
Thanks for your query and an elucidate history.

To recapitulate and explain the points noted by you:
Male / 79 - history of Crohn's in past - that your Doctor is thinking about - your history says- Constant, sharp burning pain with some swelling in the lower right side of the pelvis area - worsen by sitting upright.

Since there is a swelling, Pain is constant, sharp and burning, this need to be investigated as follows:
-Actual physical examination and clinical evaluation by a General Surgeon / Surgical Gastroenterologist.
-CT scan of the abdomen.
-Colonoscopy.
-Tests of blood, urine and stool.

This way alone the exact diagnosis can be made and the proper treatment be instituted.

May I please know a few details:
-The exact area of swelling.
-Any associated other symptoms of bloating, change in bowel habits, nausea, vomiting, fever or so ?
-Any back ache or swelling ?
-Does the pain increase on movement of the right lower limb ?
Any urinary symptoms ?
-Any history of inguinal hernia ?
- Any relation to the food ?

These may help me more to give a clinical possibility.

I hope this answer helps you to get a proper diagnosis, and get a proper further treatment. Please feel free to ask for more or if you need further clarifications if you feel there is a gap of communication.
