Question: what causes bilateral bulky ovaries in a 23 year old ,with mildly increase fluid volume. how do you treat this

Ovarian malignancy should be ruled out

Detailed Answer:
Hi dear, I have gone through your question and understand your concerns. Bilateral enlargement of ovaries can be due to polycystic ovaries or benign or malignant condition of ovaries. Since multiple follicles are not seen, therefore polycystic ovaries are unlikely.
I would suggest you to get a colour Doppler ultrasound to assess the blood flow in the ovaries, and get tumour marker CA 125 done.
These tests are done to rule out any malignant ovarian tumours.

can a 23 year old have, ovarian cancer,bacause the ovaries are bulky, is ovarian cancer asymptomatic, there are no clinical symptoms, I want you to speak about the amount of fluid ,is this normal, will it reabsorb or does she need laproscopic aspiration .what drugs she can take, or would birth control pills help her remember she have irregular menses and wants to get pregnant . would metformin help her at this stage . Thank You

Hi dear, thanks for responding. Ovarian cancer can occur at this age, though not very common. Bilateral affection of ovaries with fluid in them is concerning, and needs to be evaluated. Once the malignancy is ruled out, she can go for laparoscopic aspiration of the cyst.
Metformin is unlikely to help as it is not a case of polycystic ovarian disease.
Birth control pills can be started for treatment once malignancy is ruled out.
