Question: Doctor i'm suffering from Continuous breathing difficulty from XXXXXXX 16 2014. as first step of medication doctor prescribed medrol, vzat , levo citrazine etc.. but same breathing difficulty was there after taking medicine also.. then from 19 XXXXXXX 2014 doctor given symbiva inhaler formoterol fumarate and budesonide inhaler daily two times two puff each for two months... now I'm taking I for 1.5 months but no change in breathing difficulty. its increasing frequently. Still I've to take deep breath frequently... I sleep never faced a noicable difficulty to breath.. 3 years and 5 years before two times I got cough which extend for two to three monthss. When coughing occurs it will last for three hours.. some times it will ends bye vomiting. But this much long breathing difficulty I never faced. It seems lik this inhaler don't. Have any effect. and even i felt i have a relief when i stop inhaler for some days. now again i visited doctor and again he said technical failure in taking inhaler and said to take it for two more months and gave a tablet for seven days which contains levo citrazine and loratidine... now im taking breath like pft test and i feel like breathing problem increases too much. and i feeled increased mucous secretion in throat and chest Please help me with your reply. From your description, it is clear that you are suffering from asthma.

The mainstay of treatment of asthma are inhalers. The one you are using is also good. Or you may try seroflo or seretide. But you have to take it continuously and through proper technique. Most of the time poor control of symptoms is due to not taking the inhaler medicines properly or taking on as and when required. You have to take it even when you are not having symptoms and when full control is achieved, then it can be tapered off gradually.

You should also take montair lc and a pan 40. Avoid dust and smoke and sudden variations in the temp. Do not be tensed unnecessarily. You will soon be alright.

thank you for ur kind reply.
doctor i heared like after using inhaler we will get sudden relief. im using this for 2 months and still no noticeable change. im having difficulty every day almost every time... but never missed my sleep because of this... i was there at sharjah Uae for last three years , but used to come for vacation to XXXXXXX atleast 3 times per year and it will last to one months atleast, this time staying here for last four months and here climate is like rain one day then clear sky.. is this have any effect. Also at my home always an atmosphere of smoke is thre due to burning wood and all...... but doctor said no restriction in bathing in cold water and having cold food.

before four months i had a mild breathing problem and i had a nebulization with pulmicort and it went byt the same day itself... that time i was working in a bread factory always have good amount of dust particle...
Madam if u really feel the technique is not proper, then can we replace inhaler with nebulizer for a short period of one week. i have a nebuliser at home.

after taking inhaler im getting shivering also... and my periods which was normal earlier was too abnormal now. and i got a sever face pimples also after medication...

and i feel good amount of mucous win throat while talking and all.. but no coloration and all

my entire family is in a tension because of my breathing issue. and im not even able to concentrate in my kid... hope you will understand my tension..
i have a 2 year old kid and im still feeding him . and now he is having milk from one breast only for last one year. and kid used to sit and play in my chest before getting this problm.

You have to take controller inhalers.

Detailed Answer:

I`ll try and answer your query one by one.

There are two types of inhalers. One which give immediate relief-like asthalin or levolin. They are called relievers. They give immediate but short lasting relief. Then there are others called controllers which will control your symptoms over long period of time. These are symbicort or foracort or seroflo or seretide. These have to be taken on regular basis even when you are not having symptoms to achieve optimal control.

Yes, humid atmosphere will worsen your symptoms and smoke in any form will also aggravate your symptoms.

Very cold water bath should be avoided. Make it atleast lukewarm.

No, controller medicines are not available for nebulization. However you can take these medicines as rotacaps instead of inhalers for better effect.

I suggest you take a short course of oral steroids for 5 to 7 days AND CONTINUE WITH YOUR INHALERS. Also take a montair lc and pan 40 with it and take all precaution s as advised earlier.
