Suggest Treatment For Anxiety With Dizziness And Breathing Difficulty

Question: Hi!
I believe there is something seriously wrong with me but my doctor isn't being too helpful. I have had some bad anxiety in the past few weeks, and in the past few days I am experiencing these 'brain shake' sensations. It feels like a small motor at the base of my neck that sort of vibrates, and today I felt quite dizzy and faint. I struggled to consume food and struggled to concentrate, felt very weak. I have had blood tests and MRI done in the past few weeks but nothing was picked up. I feel like I am literally going insane, do you have any advice? When the dizziness occurs, I also have difficulty breathing/ breathe quite fast. My attention span is very short and I can't seem to concentrate properly at work. I hope you have any advice for me, thank you!

Hello,

Thanks for writing to us. I can understand your concern.

You have frequent attacks of anxiety symptoms. In between attacks you are also not feeling well. This suggests possibility of panic disorder, a type of anxiety disorder. To confirm the diagnosis detail history and mental status examination are required to be done by psychiatrist in person. However before that all necessary physical examination and investigations should be done to rule out any physical problem.

Best treatment of panic/anxiety disorder includes both medicines and psychotherapy. .

In psychotherapy I recommend cognitive behavior therapy (CBT). In mild cases only CBT is sufficient. In moderate to severe cases initially medicines and then CBT is added. And later on medicines stopped gradually.
If medicines alone are used chances of relapse are more after stopping the medicines.

In medicines SSRI group of drugs (fluoxetine, paroxetine, sertraline, escitalopram) are main stay of the treatment. I prefer paroxetine in panic disorder. Benzodiazepines group of drugs (clonazepam, lorazepam, alprazolam or etizolam) are used initially for short duration for fast symptomatic relief.
