Detailed Answer:
Thanks for the consultation at my virtual clinic HCM!
I have carefully worked through your case, and can well realize your health worries. Being your physician, I will take care all of your medical concerns today and will put my best efforts to get you out of this situation.

You have a longstanding issue of the hemorrhoids and it is the main reason for the absurd feelings and discomfort in the pelvic area. Thanks for uploading the pictures and making it easy for making a clear diagnosis and helping you fast.

You need to avoid:-
Alcoholic beverages,
sodas
caffeinated drinks
protein diet in excess
processed red meat
spices
fried fast and junk foods

Apply ProCort cream on outside edges of the anus and with the help of applicator apply it inside the rectum too.
Take Ibuprofen for reducing inflammation on glans penis and it will also avoid any pain and discomfort associated, if there.
Take Claritin by mouth for itching on glans.
You are sexually active so also got some inflammation of the penis. It is very common in sexually active teens. You should apply Neosporin cream + Hydrozole cream on the penis especially the glans area. It is mild balanitis that will get resolved soon. If not you may upload pictures afterwards.

Take plenty of water and add Citrucel Dietary fiber supplement on daily basis. Avoid constipation and if happens, you may take Lactulose syrup at night.

Sitz bath is very beneficial for you. It is to take Lukewarm water and adding a small amount of salt in it and sitting in that dipping your genitals and pelvic area in it. It will help soothe both for hemorrhoids and the balanitis.

If you feel nothing is helping you anymore you should make an appointment with a Gastroenterologist for proper evaluation and endoscopy will see if there is anything wrong with you.

Hope I answered all of your questions. Let me know if you have further clinical concerns or doubts. I will be more than happy to get back to you with expert opinion. Please don't forget to give Five Star rating to help me assist my patients in a better way; and, please don't forget to 'Close This Discussion'.

So keep a follow up to see how you doing (Always write down my name in the start of your questions at HCM & I will be with you in no time).
