Detailed Answer:
Hi.
Thanks for your query and an elucidate history.
To recapitulate: 62 years old Male - Typical Abdominal pain on lying down, before and after urination three times in the night and the stream is weak - difficult to urinate on a surfboard ( but easy and lot on laying down) - Not during the day but get some leakage - You are on ''Clonopin (Klonopin)'' for anxiety - noted about IBS , PIF, GERD Appendectomy - but not mentioned whether you are having any problem related to it - may be except for the dull lower abdominal pain 7 to 8 on pain scale - Hernia Surgery -
concerned about prostrate, bladder cancer or? ...
Thank again for a great history- (Typical Abdominal pain on lying down, before and after urination three times in the night and the stream is weak - difficult to urinate on a surfboard ( but easy and lot on laying down) - Not during the day but get some leakage) this is more classical for an enlargement of the median lobe of the prostate.
Secondly, Clonopin can also give bladder pain.
I would advise you the following:
Physical (per-rectal) examination by an Urologist.
Ultrasonography for the genitourinary system particularly so for the prostate, to assess the size of the lobes, chance of cancer, post-void (after urination) residue and so on. It will also help to get any other positive findings about other parts of the abdomen.
Blood PSA levels
Urine - routine, microscopy, culture and sensitivity.
Urologist on examination and the basic reports of the investigations will decide the further course of action, whether you need a medical management or biopsy or surgical removal of the prostate...
Please follow his advise and you will be fine.
Also discuss about Clonopin, and whether you really need it now or can be changed to another substitute
I hope this answer helps you to get a proper guideline to get an early and perfect diagnosis and proper management to get a cure.
Please feel free to ask for more if you need to or if you feel there is a gap-of-communication.
