Suggest Treatment For Pemphigus Vulgaris In A Person With Hepatitis B

Question: Hi, My mom is suffering from Pemphigus Vulgaris. At the moment she is in severe condition. She has hepatitis B (anticore body inactive) on her Liver. Does it create any issue to treat higher doses of immunosuppressive drugs?

She cannot be given high doses of immunosuppressive agents as it will enhance the risk of reactivation of hepatitis B.
A pulse dose should be more beneficial in her case , where a high dose of immunosuppressants is given within a short period of time and then there is a long gap.

Can you please give me some more idea about this pulse dose and howmuch it is going to cost in India? Do you have any idea about the availability of this service in India? I am from Andhra Pradesh. Does this facility is available in CMC Roya vellore? or any hospital in Andhra Pradesh?

Thanks for the follow up.
It is called as DCP pulse therapy.
It is in the form of injections to be given once a month.
Permanent cure is difficult but in some cases after a complete dose the disease doesn't appear for 5 to 6 years.
Yes CMC vellore should be a good option.
Just go to dermatology opd of CMC.
They should be able to help you.

From my mom's treatment hospital, i got to know that the doctors are going to prescribe "Rituximab" therapy. Are there any side effects associated with it? Do we need to take any IVIG in addtion? Does this facility available in CMC?

Rituximab is fine but it is very costly.
DCP will be much more cheaper.
Even rituximab can only control and not cure.
Yes even rituximab is available at CMC vellore.
IVIG is not required for now.
The disease will keep remitting and relapsing so don't waste much money on it.
She needs a therapy which can control her disease and doesnot hamper her day to day activity.
I advice you to visit CMC once.
