Suggest Treatment For Ankylosing Spondylitis Pain In Ankles And All Over Body

Question: Hi, my rheumatologist diaganosed me with AS with esr 92,monocytes 11.60,crp serum 53.60, I have tremendous pain in my ankle,shoulder,back,neck,knees I can't walk ,stand and sneeze I am so afraid,my doctor recommend saaz as and indocap even administered deponedrol but I have no relief the pain is so severe ,doctor is now recommended Remicade but it is too expensive is there any alternative therapy.

XXXXXX 41 and suffering from last 6 minths

Hi , I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.

What you have been suggested is the standard treatment of care.
However, i would like to know following details:
1. Any history of recurrent uveitis?
2. HLAB27 levels?
3. Have you got your Xray Pelvis done?
4. What is your R-Factor?
5. Stiffness is more in day time when you get up or remains all the day?
6. Since when are you taking SAAZ

Dear Friend.
You have Ankylosing Spondylitis.
Since you have been on SAAZ since february, i would advise you to continue it, since it takes long to prove its action.
For pain and inflammation, i advise you following:

1. Standard of care when not responding to SAAZ, Depomedrol is Infliximab injection, that has been suggested by your doctor.

2. To take Etoricoxib 90 mg once a day in night inspite of Indocap.

3. To take Tab Tramadol as on when required basis for Pain Relief. It won't help you in stiffness but would help you in Pain Relief.

4. To do daily physiotherapy for joint stiffness and better mobility.

Actually Remicade is a Monoclonal Antibody, we have other monoclonal antibodies too, but all are costly.
I would would advise you to start on the treatment i have offered to you. And in the mean time, you can contact the companies that manufacture Infliximab, you can get the medicine at very less cost as compared to market from there.
