Question: Over the last two months my shoulder and upper right arm are very discomfort able. '''''''''''''''''''''''nothing 'I have tried has worked - Tylenol arthritis strength - heating pad --- electro therapy pulsing machine

Dear XXXX

I can understand your concern.
Dear I am very sorry for your problems. As you have mentioned you have pain in your shoulder joint and arm and that is not responding to conventional therapy.

I would like to know that whether you are diagnosed with any illness such as diabetes mellitus or thyroid illnesses or any injury to that joint, As these may be related to your shoulder pain.

I would like to inform you that there are several causes of shoulder pain such as
1 . Rotator cuff injury or tear, this is a group of muscle that rotates and helps in shoulder joint movement.
2. Rotator cuff tendinitis: it is inflammation of its tendons.
3. Frozen shoulder in this adhesion gets formed between moving surfaces.
Apart from this there are few other uncommon conditions that can cause this.
In these entire conditions one thing that is common is repetitive overhead or reaching up use of arm.
Or any injury or sleeping on same side always
The initial treatments for all these are more or less same some of you are already using such as.
1. Avoid repetitive overhead activities
2. Do not does any activity that aggravates your pain
3. Never sleep pressing on that side.
4. Do not carry any heavy bag etc on affected arm.
5. Apply ice packs for about 15 minutes as many times as you can.
6. if you have learned how to do shoulder exercises then continue with that it's very effective .
7. Apply diclofenac based topical ointment it's more effective than paracetamol based.
8. Can take oral pain killers if pain is very severe.

If still pain is persisting then proper examination of shoulder joint is required to find the exact cause and if needed x ray and MRI should be done.
As you can see dear there are so many causes it may need steroid injection in your joint or few surgical intervention may be needed. So exact diagnosis is to be made of this non responding pain and that can only be made by complete history and examination.
