Detailed Answer:

Hello!

Welcome and thank you for asking on HCM!

Regarding your concern, I would like to explain that facing your clinical scenario (burning sensation and clammy skin), it is necessary to perform a comprehensive differential diagnosis.

Several causes should be sought one by one, excluding first an ischemic cardiac disorder and other etiologies.

A careful physical exam, a resting ECG, chest X ray study and some lab tests (including cardiac enzymes: CK-MB, Troponin) would properly address a cardiac ischemia.

Also a pulmonary disorder (acute inflammation/infection, pleuritis, etc) needs to be investigated.

Shingles (Herpes Zoster) may be the source of excessive chest discomfort, so a careful inspection would be necessary to rule in/out that disorder.

A musculoskeletal inflammation needs to be excluded during the diagnostic work up (costochondritis, backbone degenerative disorder, etc).

The clinical complaints characteristics are helpful in guiding all the differential diagnostic work up.

Does that burning sensation is influenced by inspiration, coughing, or posture, etc?

How long does it persist?

Did you experience associated complaints (coughing, shortness of breath, fever, etc).

Some additional medical tests could be helpful in this regards:

- exercise stress test (if cardiac ischemia is suspected),
- complete blood count and PCR (for investigating a possible inflammation),
- Backbone imagine tests (CT & MRI), if degenerative disorders are sought,
- CPK, LDH, AST& ALT for excluding a muscular disorder, etc.

You need to discuss with your attending physician about the above mentioned issues.

Hope to have been helpful to you!

In case of further uncertainties, feel free to ask me!

Kind regards,

Dr. Iliri
