Suggest Remedies For Persistent Cough In A Child

Question: Hi,

My daughter is suffering from cough since 1 Year and I have checked her from many doctors but no luck. I also diagnosed for TB related tests but nothing comes in the tests. Sometimes her ESR found high so Doctors treat like that Cough stops for next couple of weeks but issue re appeared after sometimes.
I am very much fear for this pl suggest what could be done ..

By what you quote I feel what your kid could be having viral associated wheeze or multi triggered wheeze. I have a few questions for you -

Questions:

1. How many days per month does she cough or feel breathless?

2. How many nights per month does her sleep get disturbed due to above symptoms?

3. Does she feel breathless when she runs around or plays with other kids?

4. Are the symptoms when there are seasonal changes?

5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?

6. Is the cough always associated with fever?

I want to Reply on this because the thread was shown closed:-

1. How many days per month does she cough or feel breathless?
Cough for 20-25 Days/Month but not feels breathless

2. How many nights per month does her sleep get disturbed due to above symptoms?
4-5 Nights

3. Does she feel breathless when she runs around or plays with other kids?
No even never she complains for this kind, but she wants water immediately after heavy plays.

4. Are the symptoms when there are seasonal changes?
No problem exists on all seasons.

5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
No history of Asthma but her Grandfather & Native-Grandfather had complaint for TB.
For Skin Allergy not known...

6. Is the cough always associated with fever?
Never associated with fever.
But usually cough hybrid with Sputum (Balgam-Hindi).

Hi...by what you quote I feel that your daughter might be having asthma or multi triggered wheezing. In the basic information panel you did not mention the age of your daughter. These are my suggestions for you -

Suggestions:
1. There are certainly alternative management therapies in allopathy now-a-days. Medicine has advanced a lot and not asthma is 100% controllable.
2. Inhalers are the newest management strategies for this. If I were your paediatrician I would have suggested the use of Budecort metered dose inhaler (100mcg) 2 puffs twice a day through a spacer and this is for regular use for 8 weeks. Another metered dose inhaler is Levolin and this can used as rescue therapy whenever the kid is having severe cough in spite of regular usage of Budecort. The technique is very important and very crucial for the drug to be delivered correctly to the lungs. Regularity of medicine usage also matters a lot. So do not discontinue abruptly after you notice some improvement. The technique of administering an inhaler using a spacer has to be taught to you by your doctor and these are prescription medicines. So I suggest you consult your paediatrician for this.
3. Triggers can be environmental changes/ dust/ talcum powder/ seasonal changes/ un-cleaned a/c vents/ cold weather etc....we can specifically say this is the cause - unless we observe the kid closely - best person is the parent.

Thanks for Quick Response.

I want to know is there any kind of Test/s to diagnose the asthma before staring any medicines because it our family history no asthma recorded.

Also what test\s could be done to diagnose the T.B. as her grandfather had this disease.

Also I want to tell you 1 thing that she has minor TUMOUR or ADENOID on the backside of throat these are painful when we touch them.

Hi...coming to your queries directly.

1. Asthma is a clinical diagnosis - no specific confirmatory tests. But if the kid is above 6 years age you can perform a spirometry test for the kid.

2. TB needs to be ruled out only if there is contact with a patient of TB within the past 2 years. If the point of contact has been more that 2 years, you need not work up of TB, unless there are other constitutional symptoms like - chronic fever, weight loss, appetite loss etc.

3. This adenoids has got no relation to her current respiratory problem. This is an allergic problem of the airways.
