Brief Answer:
Non pharmacological measures may be helpful

Detailed Answer:
Hi Ahl
I have gone through your query.
It needs to be evaluated to comment whether you need a surgical management or not.
I would suggest you for dietary modification to help it heal.

Increasing your daily intake of fiber by including plenty of high-fiber foods in your diet, such as fruit, vegetables and whole grains. Avoid spicy food alcohol and smoking if you do.

Avoiding dehydration by drinking plenty of water. Exercise regularly. Do not delaying going to the toilet when you feel the urge. If you use wet wipes, avoiding products that contain fragrance or alcohol as this could lead to discomfort or itching – if you use toilet paper, use a soft brand and avoid wiping too hard. Sitz bath -- soaking your bottom in a warm bath several times a day, particularly after a bowel movement, to relax the muscles in your anus. See to it that you do not strain to pass the stool.
XXXXXXX this non pharmacological measures with medicines which you take, this should be beneficial.

This provisional advice provided by me is based entirely on the input provided to me. I would suggest examination by the doctor.

I hope this suggestion might be of some help to you. Please accept my answer in case you do not have further queries.

Take care.

Detailed Answer:
Hi Ahl
It is difficult for me comment without examining you. But the usual dosage is for hemorrhoidal attack is 5 to 6 tablet daily for 4 days, and then 4 tablets daily for for 3 days. And in chronic cases it is 2 tablet daily.
