Posted on Tue, 27 Dec 2016
Question: My wife has pain in one hand for last two weeks. GP advised to for all tests which came out all well except iron (attached), sugar, hba1c, hb, thyroid are all well.
Doctor is saying that hand pain is neuropathic pain and advised medicine for a month and also 1 month medicine for anemia.
Is it ok to start these or should i consult specialist first?

Medicines advised after tests:
Cap Autrin 0-1-0 x 1 month
Tab Pregabalin Methyl cobalamine (75/750) 0-1-0 x 1 month

Yes, she can start on the prescribed drugs

Detailed Answer:
Hi XXXXXXX
Thanks for posting your query,

I have read through your query and have also viewed the attached reports.
Since you have not mentioned the details of the symptoms your wife is experiencing apart from pain, I would stick to the diagnosis made by your treating doctor- CARPAL TUNNEL SYNDROME

Carpal tunnel syndrome is otherwise known as median nerve compression. That is it is an entrapment neuropathy wherein, the median nerves passing through the forearm is covered by layers of tissues and a covering known as 'flexor retinaculum' over the wrist area. These coverings get inflamed on prolonged stress like in activities involving wrist movements; thereby resulting in the compression of median nerve lying underneath.

Therefore the line of management is as follows:
- restriction of movements by wearing wrist splints
- pain killers and anti inflammatory drugs to relieve the spasm when needed
- steroid injection for relief of symptoms
- last resort is surgery, which involves in relieving the tightened fibres, thereby resolving the compression.

From the report attached, I have seen that the iron levels are quite low, for which she requires iron supplements. he can take Cap. Autrin which is a hematinic, helps as an iron supplement. Also take diet rich in iron like liver, red meat, green leafy vegetables, pomegranate, dates etc.

Cap. Pregabalin is known to be partially effective and is considered safe in patients with CTS. The prescription given for this drug is adequate.

But I would suggest you to get a wrist splint according to her size, as the medicines alone will not give much relief.

Hope I have answered to your query,
Please feel free to discuss further or else kindly close and give your feedback.

Thanks and regards
