Question: Dear Dr. Sheppe,
My name is XXXXXXX i am on Solian 100 mg since six days and i was before during four weeks on Solian 50 mg. This time i'am writing because i would like to know how long it takes to Solian to work if i am now on 100 mg instead of 50 mg. Thanks a lot, XXXXXXX

Antipsychotics act on similar receptors as antidepressants (5HT in particular), so their time to maximum benefit is often stated in similar terms as antidepressants, about 6-8 weeks. I would wait this long before considering an antipsychotic trial "failed." However, there is an important difference between antidepressants and antipsychotics, and that is that antipsychotics act on the dopamine system, as you know. The time of onset of action with regard to the dopamine system can be much faster, as quickly as 1-2 weeks. So the answer to your question is variable among different patients, but the bottom line is that you may see some benefits quickly, as soon as 7-14 days, but it may take as long as 6-8 weeks to achieve maximum benefit.

How are you doing so far on the new dose? Any benefits or side effects so far?

Thank you very much for your answer.
After six days i feel really few results in my fixation, but for the while they are for better. I mean i am not worsening. I just have a question: when you mean that there is an effect on dopamine system, you mean agonism or antagonism? I ask this because Solian at low dosage acts also releasing dopamine and it's what i fear and, as far as i understand, what we tried with my psychiatrist to avoid with Solian 50 mg during four weeks,

I'm glad you are doing okay.

Solian at low doses does have a partial agonist effect on dopamine, but this effect is on a gradient as best we can tell. In other words, there is no known dose at which it switches from agonism to antagonism, but rather, you have to titrate the dose to clinical response. 100mg will in theory have more antagonsim than 50mg, however really what we are monitoring is not the degree of antagonism, but rather how you respond in terms of symptoms. This is why keeping me and your doctor updated with your most current symptoms is the most important thing you can do.
