Brief Answer:
Increase frequesncy of nebulisations now.

Detailed Answer:
Hi,
Thanks for posting the query on HCM. After going through your query, I would like to comment the following:

1. You seem to have been diagnosed with Bronchial Asthma. However you do not seemt to be on regular maintenance/ reliever inhaler therapy.

2. You need to visit a Pulmonologist for detailed evaluation and management of Bronchial Asthma. Investigations that may be required are Chest Xray PA view, Serum IgE levels, Blood eosinophil counts and Pulmonary function tests. Based on the evaluation your management needs to be titrated.

3. Currently you need to take Duolin/ Asthalin respules nebulisation every 4 hourly and Budate nebulisation every 8 hourly. Also in Asthma Exacerbations, Tb. Deriphylline R 150 mg twice a day can be taken by patients, under medical supervision. You are also advised to visit a Pulmonologist at the earliest so that your medications can be titrated accordingly.

4. In diet, avoid cold food, curd, cold drinks.

I hope I have answered your query. I will be glad to answer follow up queries if any. Please accept my answer if you have no follow up queries.

Dear Dr. XXXXXXX thank you for your response. I have the following queries:
1. Duolin/ Asthalin respules nebulisation every 4 hourly. (Dr. I assume you suggesting either Duolin OR Asthalin? If so, which is more effective?)
2. Budate nebulisation every 8 hourly.

Sir, can you give me in a chart /table format to avoid confusion?
7am. Duolin
11am.

When I take Budate, do I also take Doulin/Asthilin? (As it is on 8/4 hr basis)
If so, can I mix the 2 resopules in the neubolisier or not?
If not, then which one to be taken first? what is the time gap to be given between the 2 Nebulisations?

Brief Answer:
The outline of medications to be taken are given.

Detailed Answer:
Hi,
Thanks for posying the query on HCM. After going through the follow up query, I would like to comment the following:

1. The doses of medications provided in the previous post are for a diagnosis of Acute severe asthma. It is always recommended to get yourself clinically evaluated by a Pulmonologist at the earliest and the medications provided are not a substitute to clinical evaluation.

2. Till the time an asthmatic patient gets clinically evaluated by a doctor, he needs to step up his inhaled medications as described above.

3. Clarification on doses:
a. Duolin respules are preferred and they need to be taken every 4 hourly initially for a minimum of 3 days. You are expected to visit a doctor within 24 hours. The timing of nebulisation cannon be fixed since they depend on timing of last nebulisation taken. Roughly duolin nebulisation is taken 6 times a day (at regular intervals from morning to night as per your scheduled doses eg. 7 am-11am-3pm-7pm-11pm-(and if required then at 2 am)).

b. Budate nebulisation is taken at 8 hourly intervals i.e 3 times a day (Morning - afternoon - night).

c. Nebulisation treatment is generally not advocated to be taken at home and if it is required to be taken then that means that your asthma is uncontrolled.

d. The two solutions need to be taken seperately and should not be mixed.
There is no time gap required and you can take one after the other.

4. The most imprtant thing is to get yourself clinically evaluated by a Pulmonologist at the earliest since your asthma is uncontrolled at present / also to confirm the diagnosis and get the treatment titrated accordingly.

I hope I have answered your query. I will be glad to answer follow up queries if any. Please accept my answer if you have no follow up queries.

Dear Dr. XXXXXXX
Thank you for your reply. In the time gap I was able to locate the following respules with me:
1. Levolin 0.63mg
2. flohale 0.5mg
3. Budecort 200 inhaler puff.

These are from last year (not expired). However I have misplaced the doctor's prescription.
Can you suggest from these as it is late night for me to get other medicines?
