Question: I have been taking gabapentin for sleep. I have very bad insomnia and I wonder what else I could substitute the gabapentin with as it does not work very well any more.

I read your query and understand your concerns.

Although there are multiple options which can be used but considering poor response with GABA agonist molecule over time I feel we need to use something which have different mechanism of action. In that seroquel 25 mg at bedtime is one of the excellent molecule considering your age and other medications. In fact as per my personal experience I found this medicine as first choice when other medicine fails to work for sleep.

The second molecule can be trazodone 25 mg as it causes strong sleepiness side effects when used in low dose ssuch as 25-50 mg.

You may argue that the first molecule is antipsychotic and why I need to take antipsychotic for a reason as simple as sleep. I must assure you that what I am recommending you is for sleep and have little antipsychotic properties at this dose.

I have long story of insomnia pain all over my body depression and severe constipation for ten years and tried all antidepressents, vitamins etc only responded to large doses of cocktail of laxatives later i experienced fever and sore throat some times high not responded to antibiotics and toxemia and i as a doctor try to connect all together especially after excellent improvment after taking long course of antinal and flagyl in all symptoms but unfortunately temporarily i then try antihelmnth with more improvement even better than trying gluten free for 3 months for fair improvement which may be due to restriction for foods esp processed ones and fast foods for cautious but i have many relapses the only benefit is now after connection between my small intestine and my condition i used antinal and antihelminth could you please help me to find out curative

What do you recommend for HRT: Prempro or prometherine and estradiol? and at what dose? I am 56, last August I stopped Nordette BCP treatment after 5 years for peri menopausal symptoms due to developing nausea, right eye pain, sternal and shoulder pain, and increase in blood pressure for two months. My LMP was also last August. I felt a lot better the next few months with normal Bp, though my sternal pain continued resulting in a negative echocardiogram. I had return of mild hot flashes and insomnia though. Over the last several months, joint and muscle pain and fatigue, negative serology for RA, ANA, etc. with recommendation for HRT.

I had a digestive stool analysis done because I have problems digesting sugar, starchy foods etc. I end up with loose stools and insomnia - The test showed overall low good bacteria, low SIGA, and a parasite named Endolimax nana - The test also showed low candida yeast levels - but a previous test showed high - Anyways - my gastroenterologist said not to worry about the parasite - but in some places I read that it can cause problems - I try to follow a low starch, no sugar diet and have added probiotics and fermented foods for a few months but it s still the same - Would it help to treat the parasite?
