Sometime In 2015, I Started Experiencing Burning Sensation When Urinating

Question: Sometime in 2015, I started experiencing burning sensation when urinating and I went to the hospital for treatment. It turns out it was inflections and I was given antibiotics. It got better, and everything was normal again. Months after that treatment, I started getting severe fever, often burning from inside out with severe headaches. The headaches often came with weird feeling. It feelings like something (may be aunt) is moving on my head or some kind of slight shock waves are moving through the skin of my head. At the same time, I was experiencing sored, pain/arching shoulders and hands. The pains felt or still feel like burning or slight needle prickly feeling. I am also feeling very nauseous, which comes and goes.
In addition to these symptoms, I also experienced pain in my upper right side just below the rib cabbage slightly towards my back. Sometime, this pain comes down to my lower right abdomen and my lower back. When the pain is on my lower back, it can be so severe that I can’t standup quickly or can’t stand up for long. My doctor suggested MRI of the head, lower and upper back and later CT scan and ultrasound. All these came back normal. This was after series of fruitless blood work. The blood work was repeated in different hospitals and different labs but everything was normal.
I have also tested and continue to test for STDs (hepatitis A-C. Hep B, gonorrhea, HIV, Chlamydia, syphilis) but always come back negative. I was diagnosed with Chlamydia at a point, but it was treated.
Sometime in 2017/2018, I started having heart burns and sharp pains. The pains and burns often became severe in the night. Eventually, I went to ER and was diagnosed with heart inflammation/pericarditis. The inflammation was treated but sometime still feel slight pain sensation in my chest area.
I have never felt like myself since all these symptoms started. I always wake up with headache and feeling sick, body pains especially on my shoulders towards my neck down to my elbows. I used to be very confident in myself and in what I do but this sickness has made nervous and often feeling shaky. The slight agitation will cause me shivering/shaking internally and I can always feel my heart pumping and/or beating. My blood pressure has also gone up but not to the point that requires medication. One thing that I have also noticed is mental confusion. I get confuse very easily and often slightly lose my memories (hard to remember things) and I can’t focus on anything. I feel my mind is either partially blocked or so tired to do any critical thinking. It is very frustrating.
In additions to the symptoms, I experience itching in my ears, which often comes and goes. It got so bad in 2016 that I went to hospital but as usual everything was normal. Just recently, the itching extended to my throat, anal area, genital area, mouth (keeps peeling off), and general body but the itching since then has reduce but still in my ears and penis (head). I also sometime still feel painful sensation in penis especially when I feel like urinating.
I don’t know what is causing this, but I believe it is STI or STD (I will explain). Recently, I was talking to my ex-girlfriend and she indicated that she has either experienced or is still experiencing episodes of all these symptoms. She explained that she experienced severe vaginal itching for two months, which subsided but often re-occurs. She often experiences heavy menstrual periods that sometimes last for at least 10days, pain in the lower abdomen and the tummy, nausea, vomiting (vomited for two continuous months), fever, and pains in her lower back. She also experiences headache and burning sensation in her body. Recently, she complained of heart pains and burns but didn’t have money to go to the hospital after several hospital and ER visits. The last time we spoke she said, “I contemplated suicide twice over the last two weeks”, which scared the hell out of me. She said she feels very confused and disoriented. Even her friends feel she is getting mad and no one wants to understand her. Any information, I mean anything you can think about that you think may be useful is welcome. It can save a life. She is a very nice person with a good heart, very hardworking and respectful. We broke up for reasons way beyond our control, reasons I can’t put out here.
What also scares is the thought that if it is really STI or STD and if not detected early, my wife, who is very kind, loving and sweet, will also go through the same painful, frustrating life style. In fact, she has also been experiencing similar symptoms and whenever I cry because am afraid that her symptoms are similar to the mine, she will hug me and tell me am growing too paranoid. She keeps promising me she is fine and when I can see things aren’t ok. For example, she has started experiencing heart pains/burns, back and abdominal pains, body aches and severe headaches. This month, she bled heavily for 5 days, which is very unusual for her periods, yet she feels am getting paranoid.
We don’t know what the problem is but we are hoping someone out there will. Please, any ideas and suggestions are welcome.

Brief Answer:
Your symptoms are unlikely to be due to STI.

Detailed Answer:
Hi,
Welcome to 'Ask A Doctor' service.
Thanks for posting your query.

All your blood tests were normal. USG, CT, MRI were not contributory. STD panel also came as normal. There is no STI condition exists in the absence of either manifestations or blood serology.

Only, tests for HSV 1 and 2, CMV and HPV were missing. Do try to rule out these also.

Is there any pus cells in urine deposits?
What are the physical examination findings?
Are there any lymph node enlargement or changes over genitals like balanoposthitis. You don't try to rule out yourself. Please go to your physician and get examined for this.

If still no positive findings then I'd suggest consulting a Psychiatrist also.

Let me know if I can assist you further.
