Should I Give Meftal-P Along With Ibugesic Plus To Cure Recurrent Fever Every Few Hours In A Child?

Question: My LO aged 4.5 yrs old is having fever from last 3 days and its keep coming after every 4 hours . Doc recommends ibugesic plus . Should I go ahead with Meftal P also to break the fever ?
Any signs of worry ? Should we go for tests?

I have gone through the query and do understand your concern.

Most common cause of fever in this age is some viral infection and it improves with conservative treatment like paracetamol. Most of the time it is associated with a cough, cold or some stomach infection. Sometimes fever may be the only symptom for initial few days.

If fever persists longer I would like to get some investigations done like complete blood count, C-reactive protein(for bacterial infection), urine test and test for Malaria.

You can give mefenamic acid (Meftal) for fever. The dose is 6-8 mg per kg per dose and it can be given at an interval of 8 hours. Monitor the temperature and if it is persisting even after giving Meftal or it reappears before the next dose, you can give Ibugesic plus.

I would request you to consult a pediatrician for the need of investigations and prescription of above-mentioned medications.

Try to give fluid frequently to avoid dehydration. Do tepid sponging of the whole body as this works well in bringing down the temperature.

Doctor , we have done with the tests which is attached for your reference and guidance on tests.
Yesterday night the fever came at 3 am around 99 and we have given Ibugesic plus and then in morning fever came at 11 am again around 98.7 and then again 99 at 3 pm today and we again given Ibugesic plus. Now she is again at 10 pm start heating up and likely with 98.5 or 99 fever. Now what should we do ..Doc is advising now to go for Meftal 5 @3.5 ml and then alternate basis Ibugesic plus.
Please suggest based on condition as she started fever on 29th evening . Anything to worry . Please guide .

I have seen the report and it is normal for this age.
I did not find the C reactive protein (CRP) report. I usually get it done as it helps in early recognition of bacterial infection.
Even if CRP report is not there I won't get overly worried if baby is otherwise active, playful as fever is showing decreasing trend both in terms of frequency and severity and the CBC report is normal.
I hope the fever will further decrease within two to three days. I don't think meftal is needed as it is usually given in case of high grade fever (more than 100.5 Fahrenheit). You can give ibugesic plus for fever. If fever is very frequent, of high grade and not responding to ibugesic, consult your pediatrician for evaluation of cause of fever.

Thank you Sir. The update is she is still having fever and yesterday it was reported 99 F at 7 pm and again at 3 am today in morning and now again at 3 pm in afternoon.
Just wanted to check should we need to worried for Typhoid and go for test ?
Yesterday some vomittings but after ondem it settled .
Please suggest as we are very worried as today is 7th day of fever since 29th evening .

Please find attached the test of typhoid .
Please advice . Doc suggested cefixime to be continue .

I have gone through the details. The report is not very much suggestive of typhoid as the titre is 1/80 only. Still I would like to continue the cefixime as fever is still there. The urine report is within normal limit.
I would suggest you to get the blood CRP and procalcitonin level done after discussion with your pediatrician to assess the severity of bacterial infection.
The injectable antibiotic, change of antibiotic or additional antibiotic may also be needed based on the report and temperature pattern.
As I have not examined the child personally, decision about the line of management would be difficult. Sometimes it may take longer for the fever to subside and we can wait also if otherwise child is doing well.
You can take a second opinion from other pediatrician if fever is persisting.
