Question: My son bumped heads with another kid, resulting in a knot on his forehead. (see pic)
He%E2%80%99s responsive but sleepy (its also nap time). I%E2%80%99ve applied ice, given childrens tylenol and keeping him awake. Should I take him to ER for evaluation, or ok to continue treating at home?

I have gone through the query and do understand your concern.
I have seen the image which shows swelling over left forehead.
I won't get overly worried if there is no history of vomiting, abnormal movement (convulsion), weakness in any part of body, eye movement abnormality or excessive irritability. Although excessive letharginess or sleepiness is also a marker of internal head injury but not appears to be in this case.
I would suggest you to make him asleep and observe for any abnormality mentioned above.
If you find any please take him to the ER immediately.
