Question: My name is XXXXXXX and I am 49 Year old. I am female. When I travel more than 1 hour by bus, car or train in the morning , then within 2 hour uncomfortable feeling start in my forehead. And the upper part of the stomach may look like whatever is eaten is stuck. And pressing on the stomach causes belching, then upon reaching the destination, the headache starts and vomiting begins to feel. Till evening Vomiting come and if some perfume, incense sticks or bad smells are there , vomiting comes before evening. I have this problem for almost 4 years. Sometimes this problem also happens at home. Medicine: Pantosec take in the morning for 3 months also. Other medicine like sompraz-D 40 etc also taken. But the problem did not go away. Everything is fine while traveling at night. My blood report is also fine. (attached.). Please give the right decision.

I've checked those attached reports.

This may be due to some balance-related problems like benign paroxysmal positional vertigo (BPPV), weakness of your vestibular system. It may be motion sickness or vertiginous migraine.

You need to consult an ENT doctor and get your vestibular system examined. You need to perform some vestibular tests like Dix Hall pike tests, etc. With the help of these tests, we can determine if there is any weakness in your balance tests.

Accordingly, the treatment will be done. You may need to do some vestibular rehabilitation exercises to decrease your vestibular symptoms. You can start Stugeron tablet or Vertin if you are symptomatic.

Hii. I have told you few possible diagnosis about your problem. It consists of motion sickness, vertiginous migraine, vomiting due to spondylitis and some balance related problems. All these can present in the similar way you have mentioned. Taking pantoprazole is not any permanent cure but to find out the diagnosis is important. You need to consult an ent surgeon to ruke out any balance related problems or motion sickness and any medicine physician to rule out any other medical illness and migraine. You need to give some routine blood test with thyroid function tests, blood sugar. Take a x ray cervical spine anterior and lateral view and show it to medicine to rule out spondylitis as spondylitis can also present with nausea and vomiting. If you are having motion sickness you need to control it with antiemetic drug and if needed any long acting subdermal patch to prevent vomiting while travelling. You need to rule out balance related causes as sometimes balance related diseases can present with vomiting with or without vertigo.

Don't take heavy food before journey. Take breaks between journey, don't travel for long distance. Slowly it will decrease.
