Question: Good morning, Dr. Siddiqh,
Thank you for email regarding health status.
I'm sorry for the delay in getting back to you.
I have been searching for answers to avoid surgery.
Are you aware of any new research on my situation
where alternate approaches to surgery are being researched
and used with success? If so, what?
Do you believe keeping the infection at bay can halt bone erosion
and progression of cholesteatoma?
What can I do other than drugs and surgery to accomplish this?
If you don't know please refer me the name of your doctor involved
in alternative health (natural approach) on your site.

As per the books, my experience as an ENT surgeon, there is no any other alternate treatment related to your problem other than surgical management.


In your case, as a result of your age as well as other co-existing problem (tectal lipoma, charii 1 malformation) I suggest you to go for intermittent micro suction with repetitive followups with your respective treating ent surgeon.

Very nice to hear back from you as well.
I am thankful you are at the other end of the "pond" so to speak.
Regarding my health history, I don't know whether I mentioned or
not but I do have two TBI's as well as two whiplashes.
Regarding microsuction, I went to a local ENT last week who
when microsuctioning stopped very quickly as he saw this
brought great discomfort. Two months ago I experienced
an ear drum perforation. Can this be managed without microsuction?
So far the ear is now dry. Is there anything safe I can put in there
to help loosen and remove the wax? I did have microsuction twice
by ENT's since Oct.
If this may help others, I draw your attention to the following website

Answers for your queries


1. Microscopic suction can be meticulously done in your prevailing conditions of whiplash/TBI under monitored guidance with out any complications.


2. You do not have to worry about the perforation as it is one of the manifestation of the micro suction procedure. It is not necessary for you to undergo separate wax removal procedure because the procedure of micro suction itself will remove the wax.


3. As per the study related to website what you have informed, there is no such evidence based treatment by local instillation of coconut oil/vinegar. One thing I suggest antioxidant effects of vinegar/vitamin E/vitamin C/vitaminA/turmeric taken orally can have a placebo effect/ local action on the affected area.


4. You can visit your respective ENT surgeon once in 6-8 weeks for follow-up. Until, unless your ear is dry with out any discharge, do not worry. But, take care so that you do not suffer with recurrent ear infections by avoiding self cleaning of the ear, use of cotton buds, water entry, upper respiratory infections.

I do have one more question if I possibly could ask.
In the past several months since learning of the
c-toma, I have been quite diligent with reference to
infection control. The ear has been "dry" since October.
No discharge. Do you still think it wise for
me to have thoroughly cleaned now.? I had set up
the appt with the ENT for four months from now.

Thank you sooooooo much for the information regarding
supplementation.

date as I am working on a time sensitive project for work now.

I keep the ear mostly dry in the shower
but impossible not to get a little water
on the cotton ball covering it even though I put
a plastic covering over the cotton ball.

In case of dry ear, you do not require micro-suction, but you require follow-up to see microscopic changes of cholesteatoma and its involvement in affected ear.


Please continue precautionary measures to avoid cold or sinus infections. Have a regular follow-up with your treating surgeon.

1.KTP Lasers are used- where, instruments can not have direct access in removing cholestatoma between ossicles (middle ear structures), with out altering the normal anatomy.

laser surgery done in in general anesthesia (it is contraindicated in your case due to medical risks).


2. Auto immune disorders, by character, should have relapses and should respond with steroids (cholesteatoma is continuous (no relapses), will not respond with steroids).


3. White tea has antioxidant helps in early healing. It can be used as a placebo.


4. Oto-endoscopy is almost similar to microscopic suctions, where endoscope introduced in corners, where microscope can not visualize those areas.


As per your details, dry ear does not require oto-endoscopy (corners will be free).


5. Believe me, I do not have knowledge about homeopathy medication
