Brief Answer:
cigars are worse than cigarettes

Detailed Answer:
Hello,
Thanks for posting in HCM.
Cigar or cigarettes, both contain nicotine which ofcourse as we know is addictive.
But between both, cigarette is safer as it has a filter on mouth end which prevents the heavy smoke to filter out and doesn't enter the lungs. Whereas, in cigars there is none. Hence all the smoke even the heavier one and more worse one enters the lungs easily.
In a background with hypertension I will strongly recommend him to stop smoking at once. There are very high chances of developing cerebrovascular accidents (myocardial infarction, stroke) in patients with hypertension and smoking.
You can help him quit smoking by giving other sources of nicotine other than smoking which will alleviate the need of smoking and keep him calm. There are nicotine patches, losenges, chewing gums available in market. Kindly provide him with that so the nicotine need is met and he might quit smoking.
Please ask him to stop smoking as soon as possible as its not doing any good to him but only worsen things.
I hope I have answered your query well.
If you have any questions kindly ask me back.
Thank you
