Question: My son is of 6 years. He is having hypothyroidism. His Ab TPO & Ad TG are positive. His Serum 25-OH vitamin D is 38. should he go for Arachitol 3l IU Injection.

Serum 25 OH vitamin D of 38 ng/ml, is perfectly normal. There is no need to go for Arachitol 3L IU injections.
Continue with Levothyroxine at 3-5 microgm/kg/day as planned by your doctor. Final dose depends on the response to treatment and thyroid profile.

May I request to know
1. whether eltroxin to be continued life long?
2. Does prolonged medication of eltroxin cause any adverse effect?
3. Have you gone through the Anti body (Ab TPO & Ab TG) reports? What the report interpretes about the health status i.e. any future risk?
4. In fact the Vitamin D-25-Hydroxy, Serum is 38.66nmoI/L, but the endicronologist suggests for Arachitol 3Lakhs IU injections. Kindly provide a brief interepretation on the same.
5. What else can I do as the growth of my son very slow as such he is still 15 Kg weight since last 1.5 years.

I have gone through your child's reports. Although you have not posted thyroid profile, I suppose it's suggestive of hypothyroidism. The reports you posted are suggestive of Hashimoto' s thyroiditis as the cause of hypothyroidism. It's due to autoimmune damage of thyroid function leading to acquired hypothyroidism. Poor growth seen in your child is due to this because thyroid hormones are necessary for skeletal growth and many vital functions of our body.
Don't worry as it can be easily treated by regular and compliant treatment with eltroxine. There is no longer term side effects of taking Eltroxine or levothyroxine unless and until you fluctuate the dose and take overdose. Regular monitoring of thyroid functions is needed.
As far as Arachitol dose is concerned, it's the decision of your endocrinologist based on clinical assessment along with laboratory work up. Many a times clinical examination gives an edge over lab investigation. You may ask him the reason and know his point of view.

Thanks Madam for the prompt response.

I would like to get little more clarification on the following. May please grace to answer the same.
1. Is there any diet restriction and what are favourbale diets ?
2. As per the interpretaion as mentioned in Vitamin D-25-Hydroxy, Serum report < 50 is deficient, of course it also spells that levels vary with age and increased in pregnancy, may please confirm how the value 38.66nmoI/L is perfectly normal. Is this due to age factor. If so what is the ideal value of a child of this age group who is sound mean to say having no such disorder like thyroidism etc.
3. Should the child go for rigorous exercise?
4. Does this hypothyroidism any way affect the child's memory. muscular growth and potency?
5. Is there any chance of little development at the time of attaining puberty?
6. Is there any chance of total non functioning of thyroid gland in future.
7. Is there any chanse of critical health problem due to this hypothyroidism?
I may please be apologised for these long list of questions, but request to consider the same sympathetically and answer with due analysis.

Take high protein, antioxidant rich diet

Adopting a diet rich in high-quality proteins and fats, with lots of fresh, seasonal, and organic vegetables, fruits, nuts, seeds, whole grains and nutrient-dense booster foods is the shortest, straightest path to nutritional thyroid support.
An emphasis on building, through an increased amount of protein, is recommended, since lowered thyroid function reduces the body’s ability to benefit fully from the protein foods eaten.

Cold-water fish is a good choice, as it also contains omega-3 fatty acids. Tempeh is a good source of vegetable protein, and whey protein, though more processed, can be very useful. It contains immunoglobulins and L-glutamine that can help heal a distressed digestive tract.

Antioxidant-rich foods are necessary to combat free radical damage caused by the inflammatory process. Includes Vitamin A, C,E, Zinc, selenium and iodine rich food.

I mentioned the dose as normal if the unit was nanogram/ml ( you had not mentioned the unit earlier). But if it's in nanomol/l then it's deficient if less than 50 nmol/l. So, yes...your child will require Arachitol injection.

He may go for any level of exercise depending on his caliber. Once the hypothyroidism is corrected by Eltroxine, the body behaves just normally.

His further growth including puberty depends on his compliance and response to treatment.

Muscular growth, memory and potency all will be fine with good line of treatment.
Your endocrinologist will be able to guide you better on future prospects of your child as he has examined him.
