Question: hi i had a sex with my fiance.first time.i m confused that the sperm entered in vagina or not bcoz he put panis out when sperm comes.i have throid problm i m using 50 mg medicine.daily. my last period was on 17th may .2016. my peroids are irrergular comes after next 5 days almost every month that is in XXXXXXX might be my peroid comes on 22 XXXXXXX so what should i do? i dont want to be pregnant.i dont know my

Dr Purushottam Neurgaonkar here. I welcome you to HCM VIRTUAL CLINIC. I have gone through your question, and I think I have understood your concern. I will suggest you the best possible treatment options.

1 ) First of all do not panic.

2 ) As per your history, your cycles are irregular, but this time it is a bit delayed. As there is history of unprotected sex act, in view of delayed periods it is advisable to get mornings first sample of urine tested for pregnancy. In case of doubt blood beta HCG test and USG will be of help.

3 ) Although there was no spillage of semen inside the vagina, pre ejaculation secretions do contain sperms.
So even with ejaculation out side of vagina during fertile period of the cycle, pregnancy needs to be ruled out.

4 ) Even if you are pregnant, you can opt for termination of pregnancy with medicines safely till 9 weeks of the pregnancy. Plesse follow the rules of the land where you stay.And please perform the termination of pregnancy under the expert doctor's supervision .

5) It won't be wrong on my side to suggest you to get proper sex counselling done, so as to know about safe and protected sex activity. This will help to prevent the unwanted pregnancy and STDs also .

I have taken ipil as suggested by my friends. Before 24 hrs of that . So should I wait for my periods date 22 XXXXXXX Aur before that perform test

thanks for appreciating the answer and reverting back.

I would like to know about the date of intercourse, was it in the fertile period of the cycle in the month of May. And also , it would be helpful , if you can tell date of taking i pill. You are supposed to take i pill within 24 to 72 hours of unprotected sex act to be effective as emergency contraceptive.

I will still suggest to go for urine test, as i pill should not delay your expected menses so long. As mentioned , you were expecting menses on XXXXXXX 22; and it is 3 weeks past that date. If test is negative, you need not worry.

Even if the test is negative, USG will help to know the cause of irregular menses. This will help you later on .

Hi, thanks for reverting back.

As such, I pill is taken within 72 hours of unprotected sex act. This should ensure it's emergency contraceptive action. Also, it will post pone the menses due to the effects of medicine in I pill.

Now, with idea of dates of sex act and i pill, you will have to wait for menses to restart I'm a couple of weeks. Or else get urine pregnancy test done after 26 XXXXXXX

Now, USG will be of help to know the reason for irregular periods, like PCOS.
