Is Double Dose Of Rabies Vaccine Safe?

Question: I just finished post exposure rabies vaccines 4 days ago (0,3 boosters have had full set before) and today had a possible re exposure. Do I have to get another set of vaccines or am I covered?

1. Since you have completed vaccine schedule only 4 days back, therefore your antibody levels might be adequate to protect you from any probable exposure.

2. If a person has a neutralizing antibody level more than 0.5 IU/mL following previous rabies immunization then a re vaccination is not required. If your last vaccination schedule was completed 4 days back then it is most likely that you have adequate antibodies to protect you.

3. In most situations, booster is required only if the time duration since last vaccine dose is more than 30 days.

If i was to recieve two more doses would it be harmful? Also you said in most situations what would require more boosters in less than 30 days?

I have read through your query in detail.
Please find my observations below.

1. It is not harmful to take two more doses. The objective of taking boosters is to provide you with immunity to fight any rabies virus if the animal that bit you was infected with rabies. The antibodies reach a peak after 14 days following a booster. Therefore the antibodies production is still in process from the last vaccination schedule. Taking 2 more doses will further enrich the process.

2. Most people who have taken a complete course of anti rabies vaccine will surely have developed antibodies above the minimum range of 0.5 IU/mL at 30 days from the first dose.

3. There are certain high risk situations when a patient needs more attention as mentioned by World Health Organization.
These situations include the following
(i) the biting mammal is a known rabies reservoir or vector species like bat;
(ii) the animal looks sick or has an abnormal behavior;
(iii) a wound or mucous membrane was contaminated by the animal’s saliva;
(iv) the bite was unprovoked; and
(v) the animal has not been vaccinated

In above situations, it is good to take 2 boosters even if within 30 days.

4. In many countries, there are different public health regulations concerning giving booster vaccination to patients who have second exposure after taking a complete vaccination schedule as below

(i) In Srilanka, a booster is given only if the time duration since last vaccination dose is more than 8 months

(ii) In Thailand, patients reporting with re exposure only after 6 months require 2 boosters

(iii) In Phillipines, patients reporting with re exposure after 30 days of last vaccination dose require 2 booster doses.

5. Therefore you can see that 30 days is the minimum duration followed in most countries. However, if you have a re exposure that comes under the high risk category then taking 2 booster doses will not do any harm.
