default
Posted on Tue, 13 Oct 2015
Question: Hi,
I had a very irregular cycle this month. With almost no blood for first two days and very less blood for the next two days. I had a lot of blood clots this time, now that my periods are done I still have sticky brown mucus/discharge. My periods started on 17th Sept and its 23 Sept today. I took a pregnancy test on 20th but it was negative.
My last cycle was on 17 Aug, previous to that 15 Jul, previous 13 XXXXXXX previous 11 May.
Please help me understand the possible causes for such a cycle. I am on medication Glucophage, Folic Acid and Duphatson, since i have PCOS from past 7-8 years.

Brief Answer:
PCOS

Detailed Answer:
Hello,
Thanks for the query to hcm,
The cycles in your case are more or less regular with a gap of just less than 7 days, so thats still considered regular. The fact that you have been a known case of PCOS since 7-8 years now and have been on glucophage, folic acid and dupastan ( which you start after day 16 for 10 days, i believe).
As your concern is pregnancy at present , the cause of heavy clots is likely to be anovulation which is common in PCOS. Secondly hyper androgenemia in PCOS itself can cause delayed ovulation sometimes or no follicular growth at all leading to unopposed oestrogen action.
In your case it is important that you should be switched to METFORMIN 500mg twice a day, folic acid, MYOINOSITOL TABS ( EVASURE TABS one daily), and also ovulation induction should be tried with clomifene citrate 75mg from day 2- day 7 of fresh cycle and followed by follicular monitoring till ovulation occurs and then either timed intercourse or IUI should be planned.
Weight reduction is also an important factor for conception in case of PCOS as increased weight leads to increased insulin resistance which leads to increased chances of anovulation-hence no pregnancy.
