I need advice . Colon surgery was march 2014. Removed bowel tumor n was stage 3.

Taken xeloda

August 14 scan showed liver met 1.5 cm and a node .gave irinotecan alone and after 4 cycle done a scan which showd:

Liver met now 4 cm
Node grew by 0.5 cm
New sub pleural nodule

Dr said no more chemo and discharged ! We were not happy so got a sec opinion from oncologist who said chemo needed .

No chemo since dec so recent scan shows now liver met 6.7 cm plus 3 small spots on liver under 1 cm

Any other options ?

Brief Answer:
Follow the second Oncologist - please talk to Surgical Oncologist

Detailed Answer:
Sorry to know the progress,
Read and understood.
Colon Surgery in March 2014 - stage 3 colonic tumor- Xeloda - Aug 14 scan liver met 1.5 cm and a node - Irinotecan alone 4 cycles - Liver met 4 cm node grew by 0.5 cm new sun pleural nodule - Dr said no more Chemo - and discharged - Second opinion of another Oncologist says Chemo needed - No Chemo since Dec > liver met 6.7 cm 3 small spots on liver under 1 cm .
Wants to know any other options:

I would like to know a few more things before I answer your query:
When was the second opinion taken ?
Did you follow the the medications that the another Oncologist gave or just consulted him very recently.

The possible thoughts of your Doctors: Since the metastases are enlarging in size and numbers, this indicates that the Chemo is not working on this particular tumor you have.

With the situation you have informed, the other options is to follow the Oncologist and ask frankly whether any other newer Chemotherapy is useful or not.
Take a second opinion of the Surgical Oncologist whether local resection of the liver can help, if the disease is limited to a particular segment/s of the liver and not spread to any other part of the body. Also ask about the role of CT-PET scan, whether this can help in follow-up ?

Opinion was recent and he said to give oxyplatin as he seemed confident this will work .

I don't know why after 1 chemo
The previous dr just gave up ! So really he's had no chemo since august 2104 as irinotcan failed and then the gap since dec.

What is
Prognosis for this
Situation without chemo
.?

And with chemo
And ablations ?

His all liver function tests etc r normal .

They won't operate right as lymph node involved and subpleural
Nodule?

Brief Answer:
Very true.

Detailed Answer:
I think you should follow the opinion of the second Oncologist- Oxyplatin may work.
The response of the medicines do differ in every patient.

Prognosis without Chemo : with the history you have provided, it does not look good; but who knows Oxyplatin may work and the body has its own capacity to respond to the diseases and sometimes we can see results.

With Chemo and Ablations : Should be tried ASAP.

It is good that the liver function tests are normal.
Very true, secondaries at different places too become a ethical hurdle in operating, unless the reduction of the mass-load by surgery increases the chance of Chemo working better.
