Is HIV Blood Test Done 7 Days Post Exposure Conclusive?

default
Posted on Thu, 7 Jan 2016
Question: Hi Dr., I had protected vaginal sex with escort 7 days ago. She appeared very clean and professional. Condom was used end to end. Also, at the end of session (approx. 20 mins) she removed the condom. It did not appear broken. Durex was used. I did a blood test yesterday and outcome was:
"HIV Ag/Ab screening (CMIA) Non-Reactive "
What would be the risk for me? Am very worried

Brief Answer:
You are not at risk of getting HIV so don't worry.

Detailed Answer:
Dear Rjshree,

Welcome to HCM.

You had protected sex with an escort who is clean and professional. They take good precautions and would be HIV negative so the risk of getting HIV is not there. Also the CMIA Ag/Ab test result was negative so you need not to worry at all because you are absolutely free from HIV. However there is a higher risk in these always than having monogamous relation.

Thank You

Thanks doctor...but the blood test was done just 6 days after the encounter with the escort. Is this a reliable result?

Answered by Dr. Sunil Jangid (18 minutes later)
Brief Answer:
The blood test for CMIA Ag/Ab after 6 days is quite reliable.

Detailed Answer:
Dear xxxxxxxxxx,

The CMIA test for Ag/Ab after 6 days is quite reliable. You can get accurate result after 15 days with HIV Duos 4th generation test. The risk is not there in your case so be relaxed and don't worry but I will advice to stay away from unsafe practices to prevent the future risk of HIV and other STDs.

Thank you doctor. To be specific, the relationship was on wed 5pm..blood test was done on Monday following week. Result was given on Tuesday. So the specimen was around 5+ days only. Should I repeat test? I don't have any symptoms but anxiety.

Brief Answer:
You can get tested again after 15 days or one month for mental satisfaction

Detailed Answer:
Dear xxxxxxxx,

As such there is no need of any test as you are risk free but for your mental satisfaction you can go for repeat testing after 15 days or month with HIV 4th generation test to get the accurate result.

Thank you doctor. Am sorry for asking alot of questions. What I am hearing from the internet and studies is that the HIV free can only be confirmed at 3 months mark. This is why I am tensed because my test as above was done just 5 days later.

However to date I have no symptoms. Today marking 8 days since my exposure with escort. Also to let you know the escort never allows you to kiss her or whatsoever. She was just very rough in sex..but i am very sure the durex condom was intact. Only thing was that it was loosening a little but I ensured I pulled it back. The head of penis was always covered.

Brief Answer:
The antibody test for HIV needs to be confirmed at 3 months not antigen.

Detailed Answer:
Dear xxxxxxxxx,

The window period for HIV infection ranges from 15 days to 3 months during which antibodies against HIV develop in the blood that is why antibody test for HIV needs to be confirmed after 3 months of infection to be 100% sure.

The antigen detection tests for HIV are able to detect the presence infection after 15 days and HIV Duos 4th generation test detects both antigen and antibody so it is highly reliable and accurate after 15 days so don't worry and go ahead.

Thank you doctor. Am summarizing the discussion here:

On Wed, 5pm had the exposure with escort. Protected vaginal end to end.

On Mon, 8am did the HIV ag/ab cmia blood test. This is roughly 5 days after the exposure.

On Tues, 5pm received result as non reactive.

Today, is Thursday. That is 8 days after exposure, but blood test was 5 days after exposure. No symptoms apart from anxiety and stress.

Can I confirm am safe and free from HIV with the cmia test for sure? Symptoms if any would they have shown up in the last 8 days?

Thank you doctor.

Small correction the test result is 5 days after exposure not 8 days.

Will the antigen show symptoms by now if am exposed to the virus?
doctor
Answered by Dr. Sunil Jangid (3 hours later)
Brief Answer:
The risk is not there in your case so don't bother much about HIV.

Detailed Answer:
Dear Rxxxxxxxxxx,

The possibility is already nil in your case so don't bother much about HIV because you are free from infection. The antigen test will give symptoms with the infection that is not there with you so just calm down yourself and move ahead in life happily.

Thank You

Thanks for the feedback doctor. I am relieved. However, am still confused.

You mention that "The antigen detection tests for HIV are able to detect the presence infection after 15 days"
However, my test "HIV Ag/Ab screening (CMIA)" which came back as Non-Reactive was only after 4.5 days.
Can this test detect the presence of virus/infection in such a short period if I am at risk?

Sorry about asking too many details doctor, am just too worried. I was told that this test checks HIV 1 and 2.
doctor
Answered by Dr. Sunil Jangid (23 minutes later)
Brief Answer:
The CMIA test can detect the infection after 6 days but not 100% accurately

Detailed Answer:
Dear xxxxxxxxx,

The Ag/Ab CMIA HIV test can detect the presence of infection after 6 days but it is not 100% accurate even though it detects both HIV 1 and 2.

You need not to worry at all dear because you are absolutely risk free. Just relax and avoid excessive stress or tension as it is not good for your health.

Thank You

Dear doctor thanks for your encouraging words.

So, am I right to assume if indeed the escort is HIV positive, the cmia test after 4.5 days can still trace the virus and reflect in my report right? The fact that it says non reactive after 4.5 days to exposure the escort may be HIV free.
doctor
Answered by Dr. Sunil Jangid (24 minutes later)
Brief Answer:
The CMIA test can detect the virus after 5 days but not so accurately.

Detailed Answer:
Dear xxxxxxx,

The CMIA test can detect the virus after 5 days but not so accurately but again I will say you need not to bother about HIV as you are risk free. Just relax yourself and don't think too much about HIV.
