Question: Hi , My name is XXXXXXX . My age is 34 years . Planning to conceive although not enough trials through natural process .Husband age is 40 years. Having visited fertility specialist in Nov 2015, have undergone pelvic scan (detected AMH 0.48 ). Advised TB PCR and biopsy - all fine . HSG test done this month(April 2016) - tubes clear . Medication - Folic acid , calcium and for low AMH - T Ovares (2 months). Visited doctor in April 2016 , advised for IUI from next cycle and asked to continue medication. However injections from 1st to 12th day advised for IUI process . Is it really required to undergo this process to conceive and am quite nervous of the pelvic scans and other tests due to pain. will be trying for natural means this month . scan has not been done to study follicles after taking medicines for 3 months or at present.Husband tests were ok with only advise for Agglutination in sperms (medication given) . Please suggest what is best for my case .

Brief Answer:
You need to go ahead with IVF soon

Detailed Answer:
Hi XXXXXXX

I understand your concerns about infertility.

Considering your age and low AMH I don't see any reason to continue naturally or IUI. You should proceed with IVF at the earliest.

Waiting for more time will lead to exhaustion if remaining follicles and need for ivf with donor eggs.

Hence please hurry with IVF.

let me know if you need anymore help.

Hello Dr. XXXXXXX

Thanks for your reply . Feels quite disheartening to see that low AMH is so serious . If I were to start with the procedure suggested by doctor in next cycle , my concern is what is the impact of the hormonal injections on body to be taken daily until fertile period onsets. Is a pelvic scan necessary every time or is there any alternative ? (Reason being it was too painful to bear last time and the doctor I visit usually prefers it) . Shall I go for a follicular scan now or have a AMH test done (15th day today of cycle) to know if it has improved since last November as I was on medication. My weight is 58.5 kg and height 5 feet 1 inch . Tendency of weight gain(8 kgs increase in last 6 years period). What is the success rate of natural conception in my case as HSG was done recently .

Brief Answer:
Pelvic scan offers better visibility

Detailed Answer:
Hi,

Hormones won't harm. They will increase chances of getting more eggs.

Pelvic scan is better option as the visibility is very clear and crisp.

Get regular follicular growth scan done. Weight loss will help you.

Considering your age and low AMH levels chances of getting pregnant naturally is difficult.
