Question: I have been advised to start a new thread.... here goes...

I have had cramps in my calf and thigh intermittently for around 3 months. Twitching started about 2 months ago in same leg long before I was anxious about it. I have been to the GP yesterday. I am going completely insane with worry over ALSand cannot get it out of my head as I have read and re read that it can begin with theses symptoms alone. I am not coping well and the gp knows that. She checked over my whole body for any changes.. all fine... she tested all muscle strength in arms and legs.... all fine … she did reflexes ….. all fine and Babinski test all fine. I feel that I am now just waiting for something to fail . Most drs say muscle weakness first but one says it can take 6 months from the onset of twitching. My GP says no they would see something can you help I keep getting conflicting answers

Brief Answer:
Can be due to electrolyte disturbances or anxiety.

Detailed Answer:
Hello.
Thanks for writing to us
The cramps and twitching that you are having with all normal muscle strength and reflexes suggests the cause to be related to electrolyte disturbance or anxiety or thyroid disturbances or a decreased fluid intake. This can also be related to low levels of Vitamin B12 or Vitamin D3. ALS is highly unlikely with the history that you have provided
It is best to get a few blood tests done to find out any abnormality related to electrolytes, vitamin levels and thyroid levels. You can start proper medicine after that. Till then try to have plenty of electrolyte rich fluids and stay calm. Doing exercises like deep breathing exercises to calm your mind will also help.
Hope my answer is helpful. Any follow up queries are always welcome.
Regards.
Dr. Praveen Tayal.

Brief Answer:
Yes something should have been detected.

Detailed Answer:
Hello.
Thanks for writing again.
A muscle weakness or some nerve function disturbance could have been detected in case of ALS. It is not necessary that weakness comes at the end. Reflexes do show the nerve weakness.
This is not likely to be due to ALS.

Would the weakness be apparent and does the cramp becoming more frequent mean progression ? It happens every day when I am at rest the muscle starts to twitch and then cramp

Brief Answer:
Preogression can cause numbness.

Detailed Answer:
Hello.
Progression can cause a numbness, altered sensations like tingling or muscle weakness. Cramps can be related to the electrolyte imbalance.
Twitchings and cramps do not suggest a muscle weakness.

The leg does feel different like it’s heavy it’s not numb but it does tingle between the electrical shock sensations and the twitching. Is twitching in ALS constant or does It stop when u walk around would there be definitely other things happening by now ? I thought the progression was the progression of weakness

Brief Answer:
Twitching is associated with loss of muscle mass and weakness.

Detailed Answer:
Twitching in ALS is not the only symptom. Twitching is associated with spasticity and muscle weakness. It affects all the limbs and not the legs only. You need to get a few blood tests done first and rule out other more common causes like electrolyte disturbance and hormonal disturbances.
Muscle mass loss and atrophy is also seen in case of ALS which is not present in your case.

Brief Answer:
Loss of muscle bulk would have been seen.

Detailed Answer:
Yes. If the twitching that you are having were due to ALS then the associated muscle weakness and atrophy would have been evident by now and would have been detected on your routine examination.
Regrads.

Brief Answer:
Proper supplements will correct the vitamin D levels.

Detailed Answer:
Electrolyte disturbance can cause symptoms in one leg. The low levels of vitamin D can cause dizziness and weak bones. It is nothing serious. Proper supplements can bring back the levels to normal.
