CYE is 45 when it should not be more than 15, bilirubin is 45.90 when it should be below 21, lymphocytes are 8% when they should be between 20-48%. Liver enzymes are normal. Fever and jundice are present. On antibiotic Zinat for 4 days.

Brief Answer:
Patient needs further investigation and in patient care for few days.

Detailed Answer:
Hello and thanks for asking query on this forum,
Have gone through the details,
Bilirubin of patient is too high further detailed testing of the patient needs to be done.
Some tests needs to be done immediately are
Hepatitis panel test which involve Hepatitis A, B, C, D and E.
CT scan of abdomen.
If WBC go further down a peripheral smear of the blood needs to be done followed by biopsy or FNAC.
Initially you have written liver enzymes are low then its written they are normal.
Bilirubin level more then 3mg/dl is clinically significant.
Kindly visit the nearest emergency room and get the patient admitted. Such high levels of Bilirubin needs further testing like if there is hemolysis(Blood cells are breaking) or any thing grossly wrong with liver like parasitic infection or viral infection.
Detailed investigation and observation of patient is required for a minimum of 3 days.
Get the patient admitted in hospital for constant monitoring.
Follow up with reports or suggestions of treating doctor.
Hope I was helpful,
Get the patient admitted,
Follow up after that.
Regards
