Question: I am male 62 years and have IGA nephropathy for the past 13 years which is being treated with Cardace 10 mg, Stamlo 5 mg, Rosuvas 10 mg, and fish oil capsules .

In addition I have multiple thyroid nodules -no treatment, a benign prostrate (38 cc, Grade 2) – Urimax 0.4 mg medication. BP is around 130/80. I exercise daily for about 45 minutes.

My recent blood & urine test results in July 2023 are as follows:

1. Routine examination Urine - no protein, glucose or RBCs in urine.

2. Albumin Creatinine Ratio 101.56 ug/mg , Protein Creatinine Ratio 0.17

3.Serum Creatinine 1.41 mg/dl (Dec 22 -1.27, April 22 -1.22, July 21 -1.40)

4. FBS 88 mg/dl (Dec 22 -101 mg/dl, April 22 – 90, July 21 -93)

5. HBA1C - 6 % (Dec 22 - 5.7, April 22 - 5.7, July 21 - 5.8)

6. TSH 1.10 uiU/ml, Free T4 - 1.46 ng/dl, Free T3 - 3.31ng/dl

I just did a scan of my thyroid today 26 July 23-the report is attached as well as my thyroid scan in August 2022 and July 21. My latest Thyroid test results in July 23 were as follows : TSH 1.10 uiU/ml, Free T4 - 1.46 ng/dl, Free T3 - 3.31ng/dl

Queries

1. The Thyroid nodule has increased in size from 0.88 cm in August 22 to 1.09 cm in July 23-the scan report while suggesting that the nodules are benign has classified the grade as TIRAD IV -which from my reading suggests potential malignancy - please review the thyroid scan reports and give me your opinion on possible malignancy of the nodules.

2. The report mentions that there is no calcification and there is a thin halo around the nodules-does this point to a benign nodule?

3. What should be my next steps?

Thanks for the query.
I have gone through your reports.
As per your report there is a TIRADA-4 lesion in your thyroid. TIRADA-4 means it is suspicious of malignancy and you need to get a fine needle biopsy(FNAC) to confirm diagnosis.
There is slight increase in size compared to previous scan.
You can visits a good pathological laboratory to get FNAC done.

The lab made an error in the grading -it should have been rated as TIRAD 3 instead of TIRAD 4.

I have attached the revised scan report.
