Brief Answer:
Revisit medication and psychotherapy

Detailed Answer:
Hi,

Since you have a medical history of cardiac disease, unless ruled out every episode of heaviness have to be dealt very seriously.

Cardiac disorders are commonly accompanied by depression and anxiety as well in fact aggravated during angina. I see that you are already a taking few medicines to control blood pressure, second attacks and also keep lipid under control. Please do follow them.

Along with this you should visit both a Psychiatrist and Clinical Psychologist regularly to deal with anxiety and depression. There are certain medicine that can help you relieve the anxiety and depression sooner during an episode. ALong with this you have to follow things as part of psychotherpy. Let the psychologist help you with tips during the pain / heaviness.

We cannot discuss specifics here as it needs a lot of history and frequent visits to understand the nature of the illness and formulate a plan.

So I suggest Psychotherpy and proper cardiac rehabilitation is the way forward for you.
