Question: My dad is 53yrs old and has a history of diabetes and thyroid. His diabetes has gone out of control from the last one year and having been trying go back to normal without using insulin. i have attached a prescription for the tabs used since the last 3 months to get the sugar level down. His recent sugar test has showed 132 and 188 for fasting and postlunch which is better than the previous results. But however, the doc still increased his insulin tablet dosage. please refer the prescription for the latest tabs prescribed. please look into it and let us know if it advisable to use them. Are the tabs risky to use and should he use them for ever?

Detailed Answer:

Hello,

I can understand your concern. I have seen attached reports. As per latest report fasting and postprandial sugar level are 132 and 188 i.e. still these values are on higher side. So your doctor has increased the dosage. As he has hypothyroidism, so we have to adjust diabetic medication according to TSH level. So I'd suggest you to continue these medications as advised. Kindly do strict blood sugar monitoring at least three times a day. Kindly watch for symptoms of hypoglycemia like sweating, shakiness, fatigue.

Let me know please his latest HbA1C value and TSH value? Kindly do if not done earlier. Kindly follow strict diet chart after consultation with dietitian. Do regular exercises at least 30 mins a day. Kindly follow if you have any query. take care.

when you said regular blood sugar monitoring, is it like checking the sugar level before and after each meal every day? will the insulin tablets be stopped once the sugar levels come down? i l get him checked for hba1c and tsh tests. do you see any risks with his levels?

Detailed Answer:
Hello and welcome back.

Thanks for reply. After increasing dose of diabetic medications, there is a chance of hypoglycemia. So ideally we need to do sugar level monitoring daily at least before breakfast and meals. If reading shows low sugar level, then we have to adjust dosage after consultation with your doctor. Along with watch for symptoms of hypoglycemia as mentioned earlier.

Kindly do HbA1C and TSH. As hypothyroidism affects blood sugar level. So according to that, we have to titrate diabetic medications. At present, I don't think any risk with these levels, but sugar level should be under control to prevent future complications. Do regular follow up with your doctor.

Kindly follow up if you have any query. Take care.
