Question: Hi, please pass on to Radiologist expert. Uncle has bioduct cancer may of 2017, and also a small cancer in tip of pancreas. The surgeon removed it. The next three Cat Scans of area came back clean. The fourth Cat Scan read the following. Shows new soft tissue in pancreatic tall of 2.5 cm. (Cat Scan results say slight increase soft tissue region of the pancreatic tail seen posterior to the calcification on image 50, 51 and coronial image 80 and 81. (This abuts the upper pole of the left kidney and measures 2.6 x 2.3 x 2.2 cm. (Recurrent disease or localized pancreatic are in the differential. The Cancer Surgeon told my Uncle, he is not sure if this is scar tissue or not. Uncle very concerned. Told him to come back for another 2month later. Unfortunately, the second Cat scars showed that an increased size to the necrotic mass in region of the pancreatic tall is inseparable from and likely involving the adjacent stomach, anterior kidney, adrenal gland and splenic flexure measuring 5.4 x 3.6 cm in size. First question is:
1. Shouldn't the surgeon acted quickly on the cat scan that he didn't know if it was scar tissue or not.
2. According to web from different sites recommend that the Cat scan should of been combined with a PET Scan .
3. If a Pet Scan was done with the Cat scan on the forth scan, wouldn't my Uncle has a better chance of Survival. Plus would of showed Pet Scan showing images of cancer in tail of pancreas.

Last, please write back as I would understand as a patient. (Main question is shouldn't the Cancer Surgeon ordered a Pet cat, combined with a Cat Scan for more information if it was scar tissue or tumor. Thank you. Unfortunately, the Surgeon says it is just a matter of time with my uncle. He recommended Chemotherapy.. I blame the cancer surgeon. Please give me you observation, and what would you have done if you were the Doctor.

Yes as soon as the mass that was detected in pancreatic tail in CT scan a Pet CT scan should have been done just to rule ouy out cancer or necrotic mass

And one nore thing which could have been done is blood test for Ca 19.9

This blood test is tumour market for pancreatic cancer and its recurrence

I am very sorry to hear your uncles current condition


Unfortunately now it has become stage 4 and even with chemotherapy chances of survival is very less as pancreatic cancer is a very aggressive cancer

Hi Doctor, thank you. Please read additional information on cat scan on 6/10/2018 that I forgot to post. It involved 4 different area that mostly likely are cancerous. It reads the following. There is another hypo density measuring 8 mm in size. which could represent metastatic disease. Also, there is retroperitoneal adenopathy which is mild from new from previous examination suspicious for metastatic disease. Last there is a increased right lower lobe nodule a measuring up to 6 mm. Please answerer this last question, if he did a pet scan 2 and 1/2 month early, in which he didn't and started chemo a few day later let say, could the scan have showed clear of this finding of these two hypo density area, and the retroperitoneal adenopathy and the nodule in lung. Thank you very much, the question I wanted you to answerer also when they did a cat can on 3/29/2019, and saw new soft tissue in pancreatic tall of 2.5 cm. Since they didn't do a pet scan with the cat scan, shouldn't the doctor immediately did a pet scan to see if it was scar tissue or cancerous tumor to rule out. Also he would of noticed with the pet can, that is was a hot spot in tail of pancreas which was cancerous tumor, and then chemotherapy could of been started 2 and 1/2 month early, and then maybe his chances of survival would of been much better. Please let me know if you agree, and as an expert would start the chemotherapy right away. You say chemotherapy should of been started as soon as they saw the there new soft tissue in pancreatic tall of 2.5 cm. The doctor wasn't concerned because he said come back about 2 1/2 month later.

(Should the Cancer Surgeon started the chemotherapy as soon as he noticed the pancreas tail of 2.5 cm, that is what I want to know. Thank you.
