Question: Hi, my mom has got an abdominal pain for nearly couple of years. But it's kind of got worse in the last few weeks. It's mainly painful around bellybutton area and upper right side of stomach. Also she feels painful around the lower abdomen as well. She's says it's like sharp pain and she can feel that pain is going to the back as well. Last week she was complaining about feeling very cold and shivering and the abdomen pain also very bad. She said she could feel each and every bone is painful. Also she one episode of diarrhea then kind of part constipated. She couldn't go to toilet fully. She also felt nausea but didn't vomit. She feels like a tightening sensation when she stands up between her chest and stomach. We took her to the hospital and they took some tests and said her temperature and heart beat is high. It'seems 38.8 and 103. Also some of her blood tests shows high level infection. They also can trace some blood in her urine but it's not a concern they said. Her abdomen ultrasound scan , chest xray and abdomen xray also fine they said. Her blood test showed WCC is 14.5 and CRP 125. they said they need to redo some of her blood tests again and they did it again today. They did ESR, CRP, Full blood count and bone profile. They said her infection marker has gone down. Last week they diagnosed her with diverculitis. But today they said she's got heital hernia. Also she has some prolapse as well. My mom had an operation to remove her womb and one Overy 18 years ago. I'm just concerned after all these investigation what is it exactly my mom's problem. Is it diverculitis or hernia or prolapse or something serious like cancer?

Since my mom has one Overy the doctor said she did a special test called tumor mark for her remaining Overy and said the result is normal. Also the nurse did some physical examination with some gel to see if the can find any mass or bleeding. But they couldn't find any they said. They ate going to do endoscopy from mouth and back side as well.

I went through the details provided. From what I understand your mom has intermittent sudden abdominal pain located over the belly button and right side (upper as well as lower) abdomen. She has fever and nausea along with the pain. She has had multiple visits to the doctor and blood count revealed elevated counts. Ultrasound scans and other tests are normal.

To be honest without a physical examination I have limited capabilities at arriving at a definitive diagnosis. However based on the symptoms and the test results yet, I think we might be dealing with an infective condition - either related to gall bladder / appendix / diverticulosis. Cancer seems a very distant possibility as I don't have any evidence pointing towards it.

If I were her treating doctor, I might also consider a CT / MRI scan to see if we have missed out on any signs. Endoscopy should be done. I would also look to have an opinion from a gastroenterologist.

Thank you doctor. It''s just i'm a little concerned about cancer more than anything else. Especially they said they found blood traces in urine and her blood count was abnormal etc. Can I just ask you to reconfirm that the WCC is 14.5 and CRP is 125. Is that an indication of cancer? Also my mom complained about nausea and tiredness and lost 2 kg in 6 months. So i'm a little worried whether these symptoms are more of cancer than anything else. Can you please clarify doctor? Also I don't understand the reason why they are doing endoscopy. What's the purpose please?

Hi,

No, these are not definite indications of cancer. A high CRP value and elevated WBC favour more towards and infection than cancers.

Endoscopy is basically to evaluate the structural lining of the GUT - to check for defects, inflammation and other cell morphology changes (tumours). The test will allow the doctor to rule out a few things and help in the diagnosing the cause of pain.

my mom says she feels extremely tired all the time. she has no energy to even speak properly. is that a common symptom for someone having a heital hernia or diverculitis or prolapse please?

There are a few explanations to the tiredness and weakness.
1. Malnourishment from the affected condition are known to make one feel weak and tired. It should improve with correction of nutrition.
2. Active infection / inflammation also causes increased fatiguability.
3. They may be part of independent condition not related to digestive system. Diabetes, thyroid condition as well as anemia are in the first list of causes.

I can understand you are really worried about your mother and the possibility that she may be suffering from cancer. Therefore I strongly recommend she consults a gastroenterologist as discussed.
