Question: Hi, my asthma has been really bad lately. i really thought it was from pollen/dust but ive not responded to any treatment. im half way through amoxy clav amd started 40mg pred XXXXXXX yesterday, final dose tomorrow. ive been taking antihistamines, steroid nasal spray and steroid inhalers and didnt help. i have a small post nasal drip and ive noticed a pattern. for the last month ive had a lot of soda water and chilli in my food. i think this is being caused by ackd reflux. wanted some advice om management please. i will stop acidic foods etc and follow general advice for reflux online. should i stop the steroids tablets and start pepsid? should i continue the steroid inhaler and nasal spray? i will finish the antibiotics. thanks

Thanks for posting your query on HCM.

Exacerbation in asthma are well known and triggers can be anyting like a respiratory infection, exposure to dust/smoke or any other allergen, extreme variation in temperatures, post nasal drip or acid reflux. These exacerbations have to be treated with stepping up the treatment.

Dietary indiscretion could have been a trigger in your case. Now you must continue the antibiotics and oral steroids for atleast 5 to 7 days. You will have to add something for your acid reflux like a proton pump inhibitor like pantaprzole and a prokinetic like lesurdie.

Continue anti allergens like monteleukat and levocetrzine combination, regular nebulization with asthalin and budesonide respules and add an inhaler which has long acting bronchodilator like salmetrol/formetrol and a long acting steroid like fluticasone. You may also continue steroid nasal spray.

Take small meals which should not be oily and spicy

You may consult your local doctor for a presciption of above medicines.

are you able to issue a script for the antibiotics and ppis? current dose of amoxyclav is 875-125 1 tablet bid.

also, are the steroid tablets for the asthma or the reflux as im worried they will make the reflux worse?

You are taking correct dose of amoxyclav. Start other treatment asap

Detailed Answer:
I am not aware if on his platform I can make a prescription. However the dose of antibiotics is appropriate, continue it for 5 to 7 days as mentioned by me earlier also.

A short course of oral steroids for 5 to 7 days will not worsen the acid reflux much. however long duration of steroids ingestion can worsen the symptoms. In any case to control your already existing reflux you must start the treatment as advised by me asap.

hi, ive finished my antibiotics, im taking pepsid complete and being suoer carefuk regarding gastric reflux triggers but my asthma is awfuk right now. my inhalers havent been helping, ive taken either 50 or 70mg prednisone tablets today, cough suppressants, not sure what else to do and i dont want to go to ER because i will probably get covid

Everything that can be done to manage an acute exacerbation on domiciliary basis has already been done but there seems to be not relief.
You will have to visit a hospital for relief of your symptoms taking ample precautions against Covid infection. Hope you are not running fever and do not have any history being close to any Covid patient because , in that case you may have to get tested for the same.

In the meanwhile I can suggest that add Tiova inhaler also to your treatment regime. Take 2 puffs daily. You haven't specified which steroid inhaler you are taking. Preferrably take the ones suggested by me above. Similarly for your acid reflux try the medicines that I have suggested. You may find out the brands of these salts available in you location. Incae you happen to be a smoker, please quit smoking as it worsens asthma symptoms.
