Question: Hi. My husband has IBS and anxiety issues. However, over the past several months he has developed what he describes as severe muscle cramps behind his belly button - not like the low cramps you get with diarhhea (sp?). It usually lasts about 2-3 days and he literally has to remain prone. He says it is quite painful.

Hi and Welcome to "Ask A Doctor" service. I have read your question and here is my advice.
Pain in abdomen may be there with IBS , howevr when pain is severe and specially in a person who is 63, we always consider other possible causes to rule out before labeling it an IBS. So we should keenly looks for any cause of his pain.
Causes may be numerous , and is difficult to say for sure what it could be .However few possible causes that can give recurrent pain here are peptic ulcer disease , Gall bladder stones or colic , diseases of pancreas like pancreatitis etc.
Besides this diseases of the intestine itself like crohn disease may present with recurrent pain.
Rarely disease of heart and aorta ( the great vessel frim heart ) may produce such pain.
So the list is extensive , We need to see pt in details ask about other symtpoms besides pain ,
and then most of time we need investigation like ultrasound, Ct abdomen and some blood test .

pain for few days may not need so much evaluation but if this is for months we need to see it in detials.I would personally suggest take him to a gastroenterologist to assess him in details .
Hopes this help you understand your query. let me know if you have any question to ask, I would be glad to answer any further question about this. kind Regards Dr Rahman
