Hi, I Have Been Struggling To Have Bowel Movement For

Question: Hi,
I have been struggling to have bowel movement for the past 3 weeks. I constantly loose my appetite. For the first 2 weeks, i would go to the toilet , maybe once a day, and will get a small amount of stool and the feeling that i didnt get it all out. Dr. diagnosed me with an urinary infection 6 days ago and i have been taking Sulfamethoxazole Trimethoprim. After i started taking i have been having cramps in my lower right abdomen. Lately, I have been struggling with bowel movement and i would say i have a small amount every 48 hours. I know googling for most part only make us more scary. When should i pay a visit to an emergency room? What should i try to prior visiting a doctor? Perhaps a laxative? If so, what type of? I haven't noticed any blood on my stool. I have been drinking a decent amount of water every day, it hasnt helped.

I did a ct scan because i started with abdominal pain three weeks ago and then lower back pain, and it was detected calcium oxalate in my uriene. On my ct scan it says:

CT ABDOMEN: There is no urolithiasis or hydrouretereonephrosis. The unenhanced renal parenchyma is unremarkable. Without the benefit of IV contrast or no acute findings of the liver, gallbladder, pancreas, spleen, and adrenal glands. There are no dilated or thickened bowel loops. No appendicitis. Abdominal aortic caliber is normal.

CT Pelvis: There is questionable circumferencial thickening of the urinary bladder. No surrounding fat stranding. No layering fluid or free fluid. No lymph node enlargement. There are no acute osseous finding.

Impression: Questionable circumferencial thickening of the urinary bladder versus artifact. Correlate with urinalysis to exclude cystitis.

At present you are suffering from two problems at a time
1) Severe constipation-
Suggestions-
Take Fybogel husk-Fybogel sachets and Fybogel Hi-Fibre sachets both contain the active ingredient Ispaghula husk, which is a type of medicine called a bulk-forming laxative. Ispaghul is a type of fiber. It swells when it comes into contact with fluid in the gut and thus increases the volume of the feces.It increases the bulk in your stool, an effect that helps to cause movement of the intestines. It also works by increasing the amount of water in the stool, making the stool softer and easier to pass.Secondly human gastrointestinal tract is colonized by 1013-14 bacteria of 400 different species and subspecies.They are responsible for proper digestion of food.Depletion of this flora due to antibiotic therapy or age or poor food habits can lead to severe constipation and digestive problems.Take Over the counter probiotic Econorm thrice daily.
Even after 3 days if not relieved take any over the counter laxative like Lactulose 10 to 30 ml once daily for proper bowel moment.
Along with laxative you have to follow a proper diet for few weeks.The most important factor in curing constipation is a natural and simple diet. This should consist of unrefined food such as whole grain cereals, XXXXXXX honey and lentils; green leafy vegetables, like spinach, beans, lettuce, celery, asparagus, carrot. Adding some fat to meals or increasing the size of a meal such as breakfast can help create the urge to defecate. Individuals suffering from constipation should increase the amount of plant foods they consume to get more fiber in their diet. Avoid non vegetarian diet for few days.
If still not relieved you can take a fleet enema yourself.You just need to put the enema tube inside your anal canal(very easy) and push the enema fluid in bag inside.Soon you would have motion.However despite of all this if you are not relieved visit your doctor/ER.

2)Cystitis which means inflammation of urinary bladder.This inflammation along with constipation is causing severe pain.Along with antibiotic ask your doctor to prescribe you an anti inflammatory analgesic and an antispasmodic to relieve spasms.As UTI would resolve pain would go away.If he/she does not agree to prescribe you the above mentioned medicines take Ibuprofen 400 mg twice to thrice daily.

Follow up after three days.

So i've took prune juice yesterday morning and I got a watery diarrhea, about 3 hours later, i drank more this morning and had again diarrhea, in about the same amount time. But this afternoon as i drink at all, I wasn't able to have bowel movement and think a bit of abdominal pain as my appetite is back normal. Should i move to the fybogel or econorm now? Or this is a normal process and should continue to drink prune juice?

I am being able to have bowel movement about 3 times a day (much less than what i normally get). And i would say it varies from diarrhea (normally happens when i drink prune juice) and somewhat a thinner stool.

Polyethylene glycol is an osmotic laxative and would cause watery stools if taken. Even while performing colonoscopy Peg E solution is given to clear intestines so you are having loose stools because the medicine is designed to do that.

I would suggest you lower the doses of Peg. If possible buy lactose laxative and start from 10 ml once a dose.

Adjust the dose as per your convenience from 10 ml to 30 ml.
