Question: Hi, I am a 37 year old male and am a Type2 diabetic with A1C of 12 and blood sugar of 300, and I just started going on insulin temporarily a week ago. My question is, my wife and I are planning to have a second child, will this affect us or our possible future child? If so, how? And what should we do?

Diabetes will not affect the growth and development of the baby. But since diabetes has a strong genetic predisposition, your child will be prone to have diabetes later which can be prevented with the help of an active lifestyle.

Due to diabetes, you may have low testosterone levels (hypogonadism). Low testosterone levels can lead to problems which can reduce fertility such as low sperm count, erectile dysfunction and decreased sex drive.

You can plan your second baby as the risk of having diabetes is there with the first baby also and no specific treatment is needed for conception right now.

Thank you, as you said it is genetic, so is it passed through the sperm/semen? Is there anything i can do? I have also gotten balanitis, what can I do?

Yes. It is passed thru the sperms. It is in the genes. You cannot do anything to prevent it. Balanitis can be treated with anti fungal medicines. Do get proper medicines prescribed after examination by a dermatologist.

Thank you doctor. I also wanted to provide a bit of history to help us better understand what to expect with our next child. Our first son was born only at 5 lbs., because he didn't grow much after 32 weeks, and was induced delivered at 37 weeks. He was born IUGR and has had feeding difficulties since (he is now 2.5 years old). My wife complained about pain on the right side near her ribs for some time but her gyno didn't think anything of it. It does seem like maybe the baby in the belly didn't get enough placenta fluid? She also had some issues with her breasts, and also had to take medicine for thyroid during the pregnancy.

My wife would like to know if there is any blood work, urine tests, or other tests/scans she should go through to determine what the issue could have possibly been, and what things we can take care of to prevent any issues with second child.

IUGR can happen due to hypertension of mother.

Hi,

IUGR in the baby can happen due to hypertension or diabetes to the mother during pregnancy. This can decrease the blood supply to the placenta and baby thus reducing the baby's growth.

The pain in the ribs and breasts is a normal finding during pregnancy due to hormonal changes and low levels of calcium.

This can be corrected with proper supplements and do not affect the growth of the baby. You can only get her blood pressure and fasting blood glucose levels done now to rule out any such problem now.

Regular care and visits to Gynecologist are necessary during pregnancy so that any complication can be detected early.

I keep reading about preeclampsia...did she have that? She had many of the symptoms including migraines everyday, pain ip upper right abdomen, fatigue, etc. and occuring after 20 weeks of pregnancy also....this is around when I think our son stopped growing much.

Ok thanks, so there are no possible tests to see if she currently has, or could develop preeclampsia? Do you think she might have had it during her first pregnancy?

Preeclampsia causes IUGR.

There is a fair possibility that she had preeclampsia during her previous pregnancy.

But this cannot be detected now. The only way is to go for a regular checkup once she conceives.
