Question: Hi Dr. Saghafi! It’s your friend Missy from Buffalo! I’m sending you this query not only because I want you to get credit for another question, but also because our friends at HealthCare Magic wouldn’t submit my last reply to you (and I believe I had reached my allowed allotment of monthly questions lol!). So I decided to set up another account here!

I cannot thank you enough for your kindness and compassion in guiding me along in my latest round of health issues....it goes without saying that you are truly an inordinately special physician and human being...the world needs more people like you in it!

Thank you for explaining what transpired in my recovery from the general anesthesia...ugh...although my nose feels okay, my head and the rest of my body feels like I’ve been run over by a XXXXXXX truck! Wow those meds must be strong stuff! My appetite still hasn’t returned so using this as an opportunity to jump start my diet.

I like your book idea! I was also thinking about ideas while I sit here convalescing. Do you remember that book “What Color is Your Parachute?”?....it’s subject matter was all about matching your personality traits to a perfect profession. I was thinking our book could be titled (in ode to your love of ice cream) “What Flavor is Your Ice Cream?”. We could discuss the varying degrees of health anxiety (and the saving grace of telemedicine to those who suffer from it). While he average individual has the plain ole chocolate, vanilla or Neopolitan reaction to physical ailments and medical information, we have the other extreme (yours truly) who take rocky road, panda paws, Cherry Garcia, butterscotch/coconut flake/nutty flavor (I.e. approach) to it all lol!

Haha don’t get me started on local government...I’ve got a great tale to tell you about our lovely town board which approved the erextion of a 160 foot cell phone tower on land immediately adjacent to our lovely cul de sac. That fiasco transpired three months ago and I was the ring leader of the neighborhood in fighting the Town...no wonder I’m twitching!

On the medical front, I was able to obtain the MRI results from my patient portal (gotta love the portals) and I have attached it, along with copies of my old reports from 2002 and 2005. From what I can garner it looks like those small white matter changes are all that was found (I love your example of the highly trained athlete still being susceptible to the development of such changes!). Do you think there’s anything to be concerned about?

Speaking of portals, since I’m stuck at home unable to do much of anything, I logged onto the portal at the hospital where I had the surgery. Lo and behold, there was a summary of the surgery itself )interesting stuff!) along with the pathology report of my little nasal “stinker”. Turns out it was in fact a scheiderian papilloma of the exophytic type (I’ve attached those reports for your reading pleasure lmao!). I suppose the good part of it being exophytic is (from what I inderstand) the chances of it having become cancerous are basically nil, it would have grown outward rather than inverting deep into my sinuses, and the etiology is most likely only low risk HPV. In any event I’m just glad the little stinker has been decimated!

Well that’s the current Missy update. I feel bad that it’s all about me all the time lol! Someday I would love to have the absolute pleasure of meeting you my friend, you’ve been my godsend in every sense of the word! By the way Hershey PA bring about back wonderful memories of my childhood when my parents took us there to Hershey Park (long before it became commercialized). I still remember going on this little tour cart ride of the factory and the chocolate aroma!

I hope your drive home was easier than your commute in, and I hope you have a relaxing and enjoying weekend with your family!! Missy :)

I'm glad that you're able to find utility and benefit to my responses and ideas....wow....it's so nice when folks are getting helped....by services such as HEALTHCAREMAGIC....so again, A HUGE THANK YOU for your votes of confidence....

Don't worry, you're gonna get back to as good as new in short order. Things feel just a little better than "horrible" for the 1st few days but after some rest, LOTS OF HYDRATION (at least 5-7) to keep your kidneys filtering and your hemodynamic status on the slightly overhydrated side....you'll start to feel the difference....you'll see.

I suspect that by the time you read this response your appetite will start to return....but it's not entirely out of the ordinary for people's appetites to be slightly altered for a few weeks. The anesthesia has central suppressant effects on the hypothalamus.....remember the name GENERAL anesthesia....but your brain will start to get re-acclimated and you'll get back into the swing of normalcy inside of a week.

Would you believe that I'd never EVEN HEARD of the book, "What color is your parachute?" but I did look it up and discovered that I must've been living under a rock since the 1970's when it first came out and became a best seller...so I plead sheer and utter IGNORANCE.....Perhaps, since I was never quite thinking about career searching at any time (always knew I wanted to be a DOC) I just never got the initial exposure to the book and just kept my head in the sand thereafter! LOL

LOVE the suggested title for OUR book.....What flavor is your ICE CREAM? Sounds like the sequel to "All I Really Need to Know I Learned in Kindergarten Quotes" by XXXXXXX Fulghum! HA!

Would you believe me if I told you that I read the newspaper article on that cell tower you spearheaded the fight on? How could I guess that you were at the heart of that battle? And YES you are being quite insightful into hypothesizing that your fasciculations could be due to your involvement with that situation (i.e. stress trigger...well known association with muscle twitches....and QUITE BENIGN).

Now, getting to the reports my dear Missy.....I reviewed all the MRI's....and the surgical report...Truth be told, I think you could educate me much more on these Schneiderian bugaboos (as you like to kinda call em...)....or stinkers but it certainly seems like the procedure was uneventful and that you came through it with flying colors. I also read the path report as well and CONGRATS and having nothing significant at all....I'm very happy it was classified so benignly.

The MRI reports all the way back to 2002 demonstrate clinically insignificant findings and I virtually expect the read of this latest study to be perhaps a bit of progression of the WMD (white matter disease) but without evidence of any type of clinically significant disease. Your brain looks GREAT by report and from the other imaging study by photograph.

And so there you have it as the Brits might say.....and BTW, why shouldn't the MISSY UPDATES be about you? Who else shall we talk about? HAHA! And meeting in Hershey, PA seems like a great spot to hook up....if we could only find an exam room and an X-ray viewer! LOL

Hope you get some of these warm temperatures we're experiencing this weekend....supposed to get close to 60! Wow! Be well, do well.... and let me know what the MRI read turns out to be....

I’m not sure whether you saw it, but the first two reports attached (with the blue border on it) were the actual written report of the MRI I had one week ago. From what I can see, it seems pretty much the same as the 2005 report and mentions a few nonspecific areas of hyper intensity in the white matter. I actually had the films from the 2002 scan and I saw the 3mm lesion on the same; I compared it with my recent MRI and that spot looks the same.

Ah yes...my cell tower fiasco! It was quite an episode for me back in October! I am appalled that our elected town board members would approve the special use permit for the erection of such a monstrosity. You won’t be surprised, but pursuant to the Federal Telecommunications Act, state and local governments cannot deny applications for these towers based upon perceived health risks. Our Feds are obviously more interested in having their pockets lined with money from the cell industry lobbyists than they are in our health. I spoke extensively with an attorney in NYC who specializes in anti tower litigation and his parting words to me were that you could not pay him enough money to have him his family live within 1500 feet of a tower. Our house is 500 feet away....gulp. I did a lot of research into the potential health ramifications of living in close proximity to the EMF’s and despite what the naysayers will argue, it ain’t good!!

Contrary to what the town had to say, under our town code they had the right to deny the application based upon it having a detrimental impact on the property values of adjoining landowners. I’m attaching a pic of the view from outside my driveway...you won’t believe it! I think we literally just sustained a twenty percent loss in our property value. There was even more to the story, as the Telecommunications company was granted the special use permit by the Town early last year, but the erection of the tower didn’t happen until nine months thereafter. None of us even knew it was in the works until it was constructed, and by that time it was too late to commence a special proceeding against the Town in state court to challenge their decision (that has a short statute of limitations of 120
days)....probably more local officials pockets being lined to keep it under wraps. And yet another element of the story....the only “notice” we were given as adjacent landowners was “constructive” notice of the tower company’s application via publication on a small
Local newspaper which no one ever purchases or reads. We only “caught wind”’of what was happening when the tower went up; there was a town meeting for the approval of Verizon to co-locate on the existing tower; I showed up with a 20 by 20 blown up picture of the view from my driveway and shamed the town board members. I realized it was all for naught, as the darned tower was already approved, but I needed to shame the town board members for what they allowed to be done to us. So that’s all the background story to the cell phone tower which has become nothing short of a hemorrhoid to me. I’ve been on Zillow every evening looking for another home for us....I’m just not willing to take any chances with our health from living so close to the monstrosity!

Well, another jam packed day at the clinic and beyond without the time I wanted to answer your response. My apologies for being so OVER the limit....knowing how STIR CRAZY you've been at home! LOL......hopefully, you're getting back to your normal feeling self.

Also, just a clarification which I apologize wasn't clear in my last answer to you. I did read the most recent MRI results and tried summarizing that detail by simply saying that none of the MRI reports that you had attached from the past nor present were concerning for any pathological lesions or significant changes in my opinion. Of course, a better way to interpret an MRI study is in the clinical context of the patient. And so, based upon my understanding of your normal and nonfocal neurological presentation- the findings on your most recent MRI reports are not at all concerning for clinically significant findings. Also, the MRI findings of the small areas of microvascular degenerative change do not correlate to the fasciculatory activity that's been happening with you.....

As an aside, has that muscle twitching activity reduced, ceased, or is it continuing?

On the cell tower issues.....when we went looking for homes we specifically made sure that the real estate agent we wanted not to be in the same neighborhood as any device, tower, power plant, or other form of radioactive emitting entity. We didn't want the risk of the obvious problems of high tension wires going down in an electrical storm which could cause immediate damage, fires, and be electrocution risks if live wires were to fall due to storms, etc. Nor did we want the EMF radiation issues. We were very strict about those exclusions to any house we would place under consideration. If anyone would put such a device or apparatus within short or even visible distance of our house....I vowed to immediately search for another dwelling.

We also don't fertilize, use insecticide, or otherwise put down chemicals of any sort of artificial nature on the lawn, in the treebeds, nor do we stay in the house when major projects such as painting, cleaning the floors, or cleaning airducts is in progress. So believe me when I say that I fully understand and sympathize with your position and why you led the charge in this case. Politicians are just so frickin' corrupt, two faced, lieing SOB's that they represent probably the only group of individuals that I would gladly leave behind if the human race ever had to evacuate the planet rapidly for the Moon or XXXXXXX and never feel badly about leaving them behind. I figure that their cold blooded, heartless, souless, amoral personalities, replete with narcissism, corruption, and conniving agendas entirely justify allowing them to remain behind to suffer whatever terminal ill fate in store for them. Furthermore, I would not feel badly nor would my conscience at all feel soured or remorseful for watching them as the spaceship lifts off.

Do you remember the scene from POLTERGEIST back in our day when the daughter gets home after a date to find the house being engulfed by the evil spirits and the little brother screaming at his father to "Dad, let's Go!.....Leave Her....Go Dad!" when the family had all piled into their car to tear out of there? Of course, Daddy got his little baby in the car....but the way the brother felt about leaving the sister without a 2nd thought.....that's what I think we could do with VIRTUALLY EVERY STINKING POLITICIANS....I try so hard to dissuade my friends from college or even high school to PLEASE NOT GO into any type of political service position such as judge, congressman, senator, or try and get in line to be in anyone's cabinet because I am so afraid that they will be ostensibly COERCED into either being satisfied with accepting corrupted solutions to problems or even worse, endorse, become the architects of, or otherwise, support legislation allowing COUNTERPRODUCTIVE and lawless things to happen...if they wish to get ahead or even JUST SURVIVE in the circles. I'm so DISENCHANTED with our legal and law enforcement systems.....you'll have to come to XXXXXXX and we'll have to spend the better part of an entire weekend for me to explain the hundreds if not thousands of examples that underlie and justify my gross cynicism and extraordinarily high disdain for anyone who has to "convince" me of their GOOD intentions in any type of politically sensitive role....ANYBODY....

But alas, I digress yet again......

You are absolutely correct (in my PROFESSIONAL opinion) about not believing that ALL IS WELL when it comes to EMF emissions in neighborhoods. There are perfectly logical, explicable, and obvious reasons to why the USA has the highest per capita incidence of newly diagnosed Cancers and other neoplastic diseases compared to any other COUNTRY on the planet. And when you look at where the CEO's, CFO's, CTO's, CIO's, and Board Members of giant corporations LIVE.....you will see quite clearly and without having to do any analysis whatever that they live NOWHERE NEAR towers or apparatuses having to do with EMF transmissions or emissions. It's sort of like asking how the damned politicians in XXXXXXX can possibly live and survive under the highly biased and restrictive rules by which MEDICARE operate since a large portion of them are within or at ages when they will utilize such laughable insurance coverage compared to what the elderly have to pay to keep a decent policy which is not under some managed health care REGIMEN! How do ask do can they possibly pass the laws and rules that make such insurance so absolutely pathetic with their donut holes, ridiculous restrictions and exclusions on the amount of PHYSICAL OR OCCUPATIONAL therapy one can receive after a trauma or stroke, etc. etc.?

ANSWER: Because none of them actually ever intended on or found themselves HAVING to use the insurance of the peasants! They have their own private insurances...supported by taxpayer money where most actually have ROLLING ENROLLMENT PERIODS....so that they have the XXXXXXX of never losing health coverage simply on the basis of their not feeling entirely comfortable with their present plans of protection. Not so, with XXXXXXX Q. Public....

Well, my dear Missy.....once again the time is nigh.....and I hope you KNOCK 'em dead....I think you're terrific for having taking the lead in that fight....don't you let them push you around. Hey, if you absolutely need to get out of that neighborhood...there is this terrific empty lot several doors down from us....great location...great school system... XXXXXXX Stefanopoulos was a graduate of our kids' high school (now they're all either graduating or soon graduating from college)....we have terrific universities in this area, and well, you'd have Parma Neurology at your service and within much closer reach.....PLUS you could still visit the Falls every now and again! HAHAHA!!

Please look into the PEPPER PIKE, OHIO region if you're on Zillow...we would love to have you as neighbors. I can virtually guarantee that there will be NO towers or other EMF transmitting apparatuses in this area...unless the Chinese bomb and occupy our Fair City! HA!....

Well, another jam packed day at the clinic and beyond without the time I wanted to answer your response. My apologies for being so OVER the limit....knowing how STIR CRAZY you've been at home! LOL......hopefully, you're getting back to your normal feeling self.

Also, just a clarification which I apologize wasn't clear in my last answer to you. I did read the most recent MRI results and tried summarizing that detail by simply saying that none of the MRI reports that you had attached from the past nor present were concerning for any pathological lesions or significant changes in my opinion. Of course, a better way to interpret an MRI study is in the clinical context of the patient. And so, based upon my understanding of your normal and nonfocal neurological presentation- the findings on your most recent MRI reports are not at all concerning for clinically significant findings. Also, the MRI findings of the small areas of microvascular degenerative change do not correlate to the fasciculatory activity that's been happening with you.....

As an aside, has that muscle twitching activity reduced, ceased, or is it continuing?

On the cell tower issues.....when we went looking for homes we specifically made sure that the real estate agent we wanted not to be in the same neighborhood as any device, tower, power plant, or other form of radioactive emitting entity. We didn't want the risk of the obvious problems of high tension wires going down in an electrical storm which could cause immediate damage, fires, and be electrocution risks if live wires were to fall due to storms, etc. Nor did we want the EMF radiation issues. We were very strict about those exclusions to any house we would place under consideration. If anyone would put such a device or apparatus within short or even visible distance of our house....I vowed to immediately search for another dwelling.

We also don't fertilize, use insecticide, or otherwise put down chemicals of any sort of artificial nature on the lawn, in the treebeds, nor do we stay in the house when major projects such as painting, cleaning the floors, or cleaning airducts is in progress. So believe me when I say that I fully understand and sympathize with your position and why you led the charge in this case. Politicians are just so frickin' corrupt, two faced, lieing SOB's that they represent probably the only group of individuals that I would gladly leave behind if the human race ever had to evacuate the planet rapidly for the Moon or XXXXXXX and never feel badly about leaving them behind. I figure that their cold blooded, heartless, souless, amoral personalities, replete with narcissism, corruption, and conniving agendas entirely justify allowing them to remain behind to suffer whatever terminal ill fate in store for them. Furthermore, I would not feel badly nor would my conscience at all feel soured or remorseful for watching them as the spaceship lifts off.

Do you remember the scene from POLTERGEIST back in our day when the daughter gets home after a date to find the house being engulfed by the evil spirits and the little brother screaming at his father to "Dad, let's Go!.....Leave Her....Go Dad!" when the family had all piled into their car to tear out of there? Of course, Daddy got his little baby in the car....but the way the brother felt about leaving the sister without a 2nd thought.....that's what I think we could do with VIRTUALLY EVERY STINKING POLITICIANS....I try so hard to dissuade my friends from college or even high school to PLEASE NOT GO into any type of political service position such as judge, congressman, senator, or try and get in line to be in anyone's cabinet because I am so afraid that they will be ostensibly COERCED into either being satisfied with accepting corrupted solutions to problems or even worse, endorse, become the architects of, or otherwise, support legislation allowing COUNTERPRODUCTIVE and lawless things to happen...if they wish to get ahead or even JUST SURVIVE in the circles. I'm so DISENCHANTED with our legal and law enforcement systems.....you'll have to come to XXXXXXX and we'll have to spend the better part of an entire weekend for me to explain the hundreds if not thousands of examples that underlie and justify my gross cynicism and extraordinarily high disdain for anyone who has to "convince" me of their GOOD intentions in any type of politically sensitive role....ANYBODY....

You are absolutely correct (in my PROFESSIONAL opinion) about not believing that ALL IS WELL when it comes to EMF emissions in neighborhoods. There are perfectly logical, explicable, and obvious reasons to why the USA has the highest per capita incidence of newly diagnosed Cancers and other neoplastic diseases compared to any other COUNTRY on the planet. And when you look at where the CEO's, CFO's, CTO's, CIO's, and Board Members of giant corporations LIVE.....you will see quite clearly and without having to do any analysis whatever that they live NOWHERE NEAR towers or apparatuses having to do with EMF transmissions or emissions. It's sort of like asking how the damned politicians in XXXXXXX can possibly live and survive under the highly biased and restrictive rules by which MEDICARE operate since a large portion of them are within or at ages when they will utilize such laughable insurance coverage compared to what the elderly have to pay to keep a decent policy which is not under some managed health care REGIMEN! How do ask do can they possibly pass the laws and rules that make such insurance so absolutely pathetic with their donut holes, ridiculous restrictions and exclusions on the amount of PHYSICAL OR OCCUPATIONAL therapy one can receive after a trauma or stroke, etc. etc.?

ANSWER: Because none of them actually ever intended on or found themselves HAVING to use the insurance of the peasants! They have their own private insurances...supported by taxpayer money where most actually have ROLLING ENROLLMENT PERIODS....so that they have the XXXXXXX of never losing health coverage simply on the basis of their not feeling entirely comfortable with their present plans of protection. Not so, with XXXXXXX Q. Public....

Well, my dear Missy.....once again the time is nigh.....and I hope you KNOCK 'em dead....I think you're terrific for having taking the lead in that fight....don't you let them push you around. Hey, if you absolutely need to get out of that neighborhood...there is this terrific empty lot several doors down from us....great location...great school system... XXXXXXX Stefanopoulos was a graduate of our kids' high school (now they're all either graduating or soon graduating from college)....we have terrific universities in this area, and well, you'd have Parma Neurology at your service and within much closer reach.....PLUS you could still visit the Falls every now and again! HAHAHA!!

Please look into the PEPPER PIKE, OHIO region if you're on Zillow...we would love to have you as neighbors. I can virtually guarantee that there will be NO towers or other EMF transmitting apparatuses in this area...unless the Chinese bomb and occupy our Fair City! HA!....

I’m sending you this query not only because I want you to get credit for another question, but also because our friends at HealthCare Magic wouldn’t submit my last reply to you (and I believe I had reached my allowed allotment of monthly questions lol!). So I decided to set up another account here! I cannot thank you enough for your kindness and compassion in guiding me along in my latest round of health issues....it goes without saying that you are truly an inordinately special physician and human being...the world needs more people like you in it! Thank you for explaining what transpired in my recovery from the general anesthesia...ugh...although my nose feels okay, my head and the rest of my body feels like I’ve been run over by a XXXXXXX truck! Wow those meds must be strong stuff! My appetite still hasn’t returned so using this as an opportunity to jump start my diet. I like your book idea! I was also thinking about ideas while I sit here convalescing. Do you remember that book “What Color is Your Parachute?”?....it’s subject matter was all about matching your personality traits to a perfect profession. I was thinking our book could be titled (in ode to your love of ice cream) “What Flavor is Your Ice Cream?”. We could discuss the varying degrees of health anxiety (and the saving grace of telemedicine to those who suffer from it). While he average individual has the plain ole chocolate, vanilla or Neopolitan reaction to physical ailments and medical information, we have the other extreme (yours truly) who take rocky road, panda paws, Cherry Garcia, butterscotch/coconut flake/nutty flavor (I.e. approach) to it all lol! Haha don’t get me started on local government...I’ve got a great tale to tell you about our lovely town board which approved the erextion of a 160 foot cell phone tower on land immediately adjacent to our lovely cul de sac. That fiasco transpired three months ago and I was the ring leader of the neighborhood in fighting the Town...no wonder I’m twitching! On the medical front, I was able to obtain the MRI results from my patient portal (gotta love the portals) and I have attached it, along with copies of my old reports from 2002 and 2005. From what I can garner it looks like those small white matter changes are all that was found (I love your example of the highly trained athlete still being susceptible to the development of such changes!). Do you think there’s anything to be concerned about? Speaking of portals, since I’m stuck at home unable to do much of anything, I logged onto the portal at the hospital where I had the surgery. Lo and behold, there was a summary of the surgery itself )interesting stuff!) along with the pathology report of my little nasal “stinker”. Turns out it was in fact a scheiderian papilloma of the exophytic type (I’ve attached those reports for your reading pleasure lmao!). I suppose the good part of it being exophytic is (from what I inderstand) the chances of it having become cancerous are basically nil, it would have grown outward rather than inverting deep into my sinuses, and the etiology is most likely only low risk HPV. In any event I’m just glad the little stinker has been decimated! Well that’s the current Missy update. I feel bad that it’s all about me all the time lol! Someday I would love to have the absolute pleasure of meeting you my friend, you’ve been my godsend in every sense of the word! By the way Hershey PA bring about back wonderful memories of my childhood when my parents took us there to Hershey Park (long before it became commercialized). I still remember going on this little tour cart ride of the factory and the chocolate aroma!
