Question: Hi Doctor,

I am a 27 year old healthy male. I am not overweight. I do not have any medical conditions such as diabetes or vascular disease, etc. I eat healthy (plenty of fruits and vegetables). About 4 months ago, I woke up one day and felt that my penis felt "dead". I immediately worried and the first thought that came to my mind was that I have erectile dysfunction (I don't know why I immediately panicked and thought this). On my drive back home I was EXTREMELY stressed. I was impatient, I was stomping my feet in my car, yelling, pulling on my hair, on the verge of tears, I thought my erections were gone forever. I tried masturbating when i got home and i could not get an erection at all. I had anxiety all throughout the night. Trying to sleep but having all these thoughts run across my mind were making it hard to sleep. I would get out of bed and walk around the house thinking if I should go to an ER. I went back to back but got up other times throughout the night to see if I should go see a doctor. I went to urologist the day after and he told me that it is not possible to have erection problems at 26 years old and blamed it all on my anxiety. He did not do any tests. He just did a physical examination of my penis for about 10 seconds and said everything looks fine. Prior to this whole thing happening, about 2 weeks before I noticed my penis was numb I had found out that I had phimosis my whole life. I basically was never ever to retract my foreskin and never even knew that other uncut men were able to do so. I always thought that uncut just meant you had foreskin covering your penis at all times. Once I found out about this I was super depressed and felt like I had some sort of defect. I read that only less than 1% of uncut men would not be able to pull their foreskin back by age 18 so I felt extremley stupid and damaged knowing that I'm 26 and not only could I not pull my foreskin back but I never even knew this was a thing. Leading up to my doctor visits I was thinking about this a lot. Thinking that I am not normal and felt very low. I was constantly googling my symptoms and phimosis was always on my mind. Once I saw my doctor he gave me a cream to put on my foreskin and it worked! I was now able to retract all the way back when my penis was flaccid but still had difficulty pulling back when erect. I was still a little depressed because I thought "what if this is al I will ever be able to retract?". I also noticed one day I woke up to no morning erection which made me a little startled but I didn't think much of it. Now, I only get morning erections about 4 or 5 times a week. Is that due to anxiety? These past 4 months my life has been consumed with doctor visits and researching online as well as constantly thinking and worrying about my erections. One second, I am able to get an erection easily (during sex and masturbation), the other second it is difficult (during sex and masturbation). I went to about 3 doctors and even asked 2 urologists online to about my problems and they all told me its because of my anxiety and that I should meditate and see a psychiatrist. They didn;t even do any tests to see if my penis arteries or nerves are damaged. They said it isn't possible to damage any part of your erection muscles or nerves without having other symptoms or having a serious medical condition that would lead to it. I was a little annoyed because, yes, I did have anxiety over these past 4 months regarding my erections but would my anxiety be THAT bad where it would be causing ED? Would lack of morning erections also be caused by anxiety or is this an indication that this may be a physical problem? I don't really think it is a physical problem either because after reading up on how erections work in the body I feel like there is no way I have any of the conditions that would lead to physical ED such as diabetes or peyronies disease, etc. I requested for my docotrs to do an ultrasound but they refused and told me it is all a psychological problem. If it were a psychological problem wouldn't it all be short term? I am not anxious ALL the time. This is absolutely normal. You gave been to urologist and nothing comes out. Even you have morning erections so I don’t think there is very serious problem.

This is due to anxiety only. You developed a thought and this thought got fixed and this causes severe stress which resulted in poor erection. Even if you have no other symptoms of anxiety even then you can have such presentation.

You can visit a psychiatrist and get proper evaluation done. This can be treated easily.

This might be a stupid question but will the the tentex forte have any side effects?

Also, how does anxiety work when it comes to psychogenic erections? Right now I see myself really happy sometimes and not worries but other times I am really depressed and worrying a lot about my erections. Does anxiety and the physical side effects of ED stay in your body until my anxiety about erections is gone completely? Or would I get my erections back if I am just temporarily not feeling my anxiety. Basically, what I am trying to ask is how long will it take to go back to how my erections were before if I control my anxiety? I've also been having premature ejaculation sometimes when I masturbate and have sex. Since the premature ejaculation only occurs about 30% of the time and regular ejaculation time rest of the time is this an indication that the premature ejaculation is psychogenic as well? so i made an appointment with a counselor for next week. Dear you are not understanding my point. If the drug improves your erection then this means that the blood supply of penis vessels in normal. Even in diagnostic tests for incompetent valves some people use Sildenafil instead of injections. This is normal.

In elderly the issues are different, like hormonal changes, hypertension, hyper-lipidemia, diabetes etc. If the blood supply is defective then Sildenafil doesn't work.

You are young and otherwise healthy, we can rule out other causes easily. This is why I am asking you to take this drug once. This is not for your treatment, this is to check the blood supply.

I did take Sidefel on the second day I had this problem and I got an erection but I don’t know if that was because of the drug or because I naturally was aroused.

I already occasionally get good erections without taking these pills though. What if the problem is a vein or artery issue and not a blood supply issue?

By "this drug" you mean Tentex forte or Sildenafil? I ordered the tentex drug and it should arrive on Sunday. how often should I be taking this and in what quantities?

I went to my urologist again today but last night the quality of erection was not good while I was trying to masturbate. This may have been because I already ejaculated 3 times in those 24 hours but nevertheless it still freaked me out. My urologist said that there is nothing wrong with me physically but he didn't even take any tests. He just sat there and talked to me. It just astonishes me that all doctors haven't taken any tests to determine that something is physically wrong with me. They all just label it as anxiety without doing any proper test. My urologist today said an ultrasound would be a waste of money since nothing is wrong with me and also said that it is not possible for there to be a circulation problem even when i told him that I am not getting frequent nocturnal erections anymore. I can't imagine there being any other medical condition that would give doctors the same reaction as erection issues. For example, if I went in and told them that I'm a 27 year old healthy man but i feel like i am having liver or kidney problems I don't think any doctor would say "it's just your anxiety, there's no way possible for a 27 year old healthy man to have liver or kidney issues" which is basically what they have all been saying about my erections. I mean....is it seriously not possible at all that something is wrong with my heart and the way it is circulating blood to my penis? Sometimes my erections feel strong and rigid but other times it is very difficult to get a the same erection I want no matter how hard I try. I may just be ignorant how on earth would anxiety be causing all of this? I am nervous that even when I begin my counseling that they'll just numb my mind and convince me that whatever is going on now is totally normal and I shouldn't expect to go back to how my erections were before.

But if there was any significant flow issues in you the urologist would have advised tests to you. So stop worrying for this. Everything is alright.

Hi Doctor, I am a 27 year old healthy male. I am not overweight. I do not have any medical conditions such as diabetes or vascular disease, etc. I eat healthy (plenty of fruits and vegetables). About 4 months ago, I woke up one day and felt that my penis felt "dead". I immediately worried and the first thought that came to my mind was that I have erectile dysfunction (I don't know why I immediately panicked and thought this). On my drive back home I was EXTREMELY stressed. I was impatient, I was stomping my feet in my car, yelling, pulling on my hair, on the verge of tears, I thought my erections were gone forever. I tried masturbating when i got home and i could not get an erection at all. I had anxiety all throughout the night. Trying to sleep but having all these thoughts run across my mind were making it hard to sleep. I would get out of bed and walk around the house thinking if I should go to an ER. I went back to back but got up other times throughout the night to see if I should go see a doctor. I went to urologist the day after and he told me that it is not possible to have erection problems at 26 years old and blamed it all on my anxiety. He did not do any tests. He just did a physical examination of my penis for about 10 seconds and said everything looks fine. Prior to this whole thing happening, about 2 weeks before I noticed my penis was numb I had found out that I had phimosis my whole life. I basically was never ever to retract my foreskin and never even knew that other uncut men were able to do so. I always thought that uncut just meant you had foreskin covering your penis at all times. Once I found out about this I was super depressed and felt like I had some sort of defect. I read that only less than 1% of uncut men would not be able to pull their foreskin back by age 18 so I felt extremley stupid and damaged knowing that I'm 26 and not only could I not pull my foreskin back but I never even knew this was a thing. Leading up to my doctor visits I was thinking about this a lot. Thinking that I am not normal and felt very low. I was constantly googling my symptoms and phimosis was always on my mind. Once I saw my doctor he gave me a cream to put on my foreskin and it worked! I was now able to retract all the way back when my penis was flaccid but still had difficulty pulling back when erect. I was still a little depressed because I thought "what if this is al I will ever be able to retract?". I also noticed one day I woke up to no morning erection which made me a little startled but I didn't think much of it. Now, I only get morning erections about 4 or 5 times a week. Is that due to anxiety? These past 4 months my life has been consumed with doctor visits and researching online as well as constantly thinking and worrying about my erections. One second, I am able to get an erection easily (during sex and masturbation), the other second it is difficult (during sex and masturbation). I went to about 3 doctors and even asked 2 urologists online to about my problems and they all told me its because of my anxiety and that I should meditate and see a psychiatrist. They didn;t even do any tests to see if my penis arteries or nerves are damaged. They said it isn't possible to damage any part of your erection muscles or nerves without having other symptoms or having a serious medical condition that would lead to it. I was a little annoyed because, yes, I did have anxiety over these past 4 months regarding my erections but would my anxiety be THAT bad where it would be causing ED? Would lack of morning erections also be caused by anxiety or is this an indication that this may be a physical problem? I don't really think it is a physical problem either because after reading up on how erections work in the body I feel like there is no way I have any of the conditions that would lead to physical ED such as diabetes or peyronies disease, etc. I requested for my docotrs to do an ultrasound but they refused and told me it is all a psychological problem. If it were a psychological problem wouldn't it all be short term? I am not anxious ALL the time. There are some times that I feel good and optimistic but then when I wake up the next morning and see I don't have morning wood i go back into feeling like something is wrong with me. As a psychiatrist, do you think these problems are all due to anxiety? I've had anxiety before with work or a stressful relationship and never had erection problems. What would make the anxiety I m going through now any different and be causing me physical erection issues? Wouldn't I also have other physical symptoms due to the anxiety such as a fast beating heart or fatigue or something? Thank you!
