Question: Hey doctor so last week I had a case of dermatitis on the shaft of my penis. I was prescribed a hydrocortisone 1% strength cream by my doctor which cleared it up within a week. However where my penis was sitting on the shaft it was touching the scrotum after I had applied the cream and now both sides of my scrotum are really red and it kind of itches and burns the scrotum skin. I visited my doctor again today and she got me to do an ultrasound of my testicles which was fine and also urine test to check for infection which I will get back tomorrow along with prescribing me Hydrozole(Anti-fungal/steroid cream) to apply to the scrotum skin. I also did have a fungal infection of my glans last month which was confirmed by a swab and the re swab didn’t show any infection either. My main concern is that I am leaving the country in 48 hours so I was wondering what you think it could be do you believe it sounds like maybe jock itch the rash is just bright red in nature and not really dry or scaly or could it be dermatitis however I did try to use just the steroid cream on there the last 2 days and it didn’t help at all so now I’m using the combo. What else could it be do you think if the ultrasound and urine test come out normal would I be able to manage for 2 weeks overseas with it if it were a fungal infection or a skin issue?

Also to add the redness on the scrotum was originally only on the right side but it has now spread to the left side of the testicles as well.

Thanks for the query.
The signs and symptoms are more likely suggestive of something called red skin syndrome (RSS) which is nothing but a reactionary eruption to topical steroids. Fungal infection or jock itch is unlikely as it will cause severe itching and a white hue will be there, redness and burning sensation will not be there.
As of now stop the steroid combination ointment. You can apply plain antifungal creams or emollients like Vaseline for symptomatic relief.
Nothing much to worry and you will be fine in five to six days.
If symptoms persist better to consult a dermatologist and get evaluated.

Hey doctor thanks for the answer I just had a few more questions. I got back the ultrasound, urine test and a blood test to test for infection and they all came out perfectly normal. However I am just wondering why I’ve had a reaction to the steroid now but I have used it in the past quite recently in the last couple of months with no issues what do you think the reason for that could be? The skin just feels really dry and it looks/feels red and irritated. My GP said to wait 3-5 days to see if it would get better and if not apply the anti fungal cream to see if it will improve. I have also had the full range of STI tests in the last couple of months which came back normal and I have long term girlfriend who I’ve been with the last 4 months who also doesn’t have anything. I did get a swab done today also for HPV but I’ve had the vaccine when I was 15-16 years old so I doubt it is that do you think most likely I just had a bad reaction to the steroid or a fungal infection? Can these things be life threading as well if untreated for a couple of weeks? I am just heading over seas for work for 2.5 weeks.

Hey doctor so just a follow up so the skin has now calmed down and I don’t have any itching or burning symptoms anymore however it is still red especially after a shower and it looks like it’s quite dry as there’s some white peeling go on. Is this a sign that is healing do you think should I just keep leaving it as it is for now? I also did a blood test which showed no signs of infection so I don’t think it is that more so dermatitis possibly?
