Question: Hey doctor,

So currently I am away overseas on a holiday and I have been having some issues health wise. I have had these white patches on the shaft of the penis and also Smegma on the head of my penis that doesn’t seem to wash off with water very well in the shower. I don’t really have any other symptoms other than that so I thought it may be a fungal infection as I have had them in the past. I have been applying Canesten Clotrimazole Anti-fungal Cream to the effected area of the penis the last day and half but I have noticed a slight burning sensation and a bit of itchiness from the cream. Is this normal with this cream as a side effect or do you think it may be making things worse? The country I travelled to which is Paris is also in winter and I came from a hot summer environment back home. How many days should I give the cream to see if it works?

According to your symptoms of white patches, redness and irritation it can be related to yeast infection.
Clotrimazole is the right treatment for it and it is good you are using it.
Use it once daily before bed after skin is cleaned and dry. The burning and irritation of skin is probably caused by clotrimazole usage. Remember to use it only once . If you have used it for three days you should stop using it now.
The warmness and sweating may have contributed to yeast formation so remember to keep yourself washed and dried perfectly before getting dressed.

Are you sure once daily is sufficient it says apply 2-3 times daily on the packaging until the symptoms go away and then for a further 14 days or is this more so for infections that aren’t on the foreskin and glans? In the past they always prescribe it as a twice a day treatment as well.

I have attached images of the cream as well let me know what you think yesterday I applied morning and night but I can switch to once daily for 3 days instead if that might be better? If it doesn’t clear up after 3 days should I just stop using completely? I have noticed so far the cream made the area a bit more red especially the foreskin but I think the whiteness and flaking Smegma has been reduced.

Also to add I am staying in Paris at the moment so it is very cold compared to where I am from in Australia. Do you think maybe the long flight here and weather changes caused the infection possibly or could it maybe be just dry skin from the cold weather?

Thanks for that doc I will apply once daily at night time however one other thing does the Clotrimazole cream I am using effect the use of latex condoms I’ve read that it can? How long of a gap should I have between using the cream and having sex?

Thanks for that doctor good to know just one further question I have. I seem to be getting these fungal infections frequently over the last couple of months do you think it’s because of my partner she said she doesn’t have any symptoms? Also I have been having dermatitis on and off on the shaft of my penis skin and also the scrotum skin which is very dry and it would go bright red some times. I’d never actually had these problems until I started having regular sex with my partner but we’ve both been tested and have nothing at all. I only slept with one other girl this year which was back in August before I met my partner which was in September and I again had been tested with both blood and urine at the time and also after the 3 month mark of having sex the first time which came out clear for anything so I am unsure why I am getting dermatitis and fungal infections?Could it be something to do with the latex condoms or just my system reacting in a weird way? In the last 2 weeks I haven’t used any condoms or had sex at all since I’ve been away in Paris but I am still have symptoms especially the redness/dryness from the dermatitis and then the fungal infection too.

You should do secretions culture, test if the white smegma secretions you were complaining.
The culture will show which antifungal medication can be used to cure it since it may be a recurrent infection.
Also your partner may need to do vaginal secretions smear to check for it. If it results positive she may need treatment too.
I believe she is transmitting it to you.
